上颌快速扩弓对龈沟液中IL-1β及MMP-8水平的影响 被引量:3

Effect of Rapid Maxillary Expansion on the Level of IL-1β and MMP-8 in Gingival Crevicular Fluid

摘要 目的:研究上颌快速扩弓前后龈沟液(gingival crevicular fluid,GCF)中白细胞介素-1β(interleukin-1β,IL-1β)及金属基质蛋白酶-8(matrix metalloproteinases-8,MMP-8)水平的变化,及其与上颌牙周组织改建的关系。方法:34例需上颌快速扩弓的正畸患者,随机分为对照组(戴入Hyrax扩弓器但不加力)和实验组(持续扩弓1周),各17例。分别在扩弓前(基线)、扩弓后24h、1周,保持1、3、5、7、9周时进行临床检查,并检测GCF量及IL-1β、MMP-8的水平,从而进行统计学分析。结果:与基线比较,实验组GCF量、IL-1β及MMP-8水平呈动态变化,扩弓后24h开始升高(P<0.05),扩弓后1周达峰值(P<0.001),此后逐渐下降,但一直保持较高水平(P<0.05)。组间比较显示,整个实验过程中,两组间临床指标差异无统计学意义,从扩弓后24h至保持7周,实验组GCF量、IL-1β及MMP-8水平与对照组比较有统计学差异(P<0.05),而至保持9周时无统计学差异。结论:上颌快速扩弓过程中,GCF中IL-1β及MMP-8水平发生了动态变化,在一定程度上可以反映快速扩弓后牙周组织的改建。 Objective:To investigate the effect of rapid maxillary expansion on the level of interleukin-1β(IL-1β) and matrix metalloproteinases-8(MMP-8)in gingival crevicular fluid(GCF),and to study the relationship be-tween those changes and periodontal tissue remodeling.Methods:34adolescent patients who need rapid maxillary expansion were selected,and were randomly divided into two groups:the control group(placement of Hyrax appli-ance,but no activation,17cases)and the experimental group(activate for one week,17cases).Clinical data and GCF samples were collected for 8observation periods:before activation(baseline),24-hour and one-week after activa-tion,one-week,three-week,five-week,seven-week,and nine-week in retention.The level of IL-1βand MMP-8were detected and analyzed.Results:Compared to baseline,the GCF volume,the IL-1βand MMP-8 level elevated significantly after 24-hour activation in the test group(P0.05),and peaked after one-weak activa-tion(P0.001),then displayed an upward trend,but still remained at higher level(P0.05).Compared to the control group,the GCF volume,the IL-1βand MMP-8level of the experimental group showed significant differ-ences from 24-hour after activation to seven-week in retention(P0.05),but no significant difference at nine-week in retention.Conclusion:The level of IL-1βand MMP-8in gingival crevicular fluid experienced dynamic changes,which might reflect the periodontal tissue remodeling during rapid maxillary expansion.
