MRV在矢状窦旁脑膜瘤术前评估中的应用价值

Preoperative Evaluation Value of MRV for Diagnosing Parasagittal Meningioma

目的探讨MRV对矢状窦旁脑膜瘤术前评估的临床应用价值。方法对10例矢状窦旁脑膜瘤的患者进行MRI平扫加增强以及MRV检查。观察肿瘤的部位、大小、对上矢状窦的侵袭程度以及代偿性静脉侧支循环通路情况,并与术中记录进行比较。结果 10例矢状窦旁脑膜瘤病例中,7例瘤体与一侧静脉窦壁外层附着;3例瘤体长入窦腔内,导致窦腔完全闭塞,上矢状窦周围出现代偿性静脉侧枝循环通路。MRV所见与手术结果完全相符。结论 MRV可显示肿瘤侵犯上矢状窦的程度和静脉侧枝循环通路情况,有助于选择手术入路、制定手术方案和术中处理好肿瘤与血管的关系。

Objective To investigate the preoperative evaluation value of MRV for diagnosing parasagittal meningioma.Methods To observe tumor＇s location and size,the degree of tumorous erosion to superior sagittal sinus and the situation of collateral venous pathways,10 patients with parasagittal meningiomas were examined preoperatively with routine MR examination,enhanced scan and MRV,and compared with intraoperative findings.Results 7 cases of neoplasm adhering to the outer layer of venous sinus,3 cases growing into and blocking it completely on the images of 10 cases.Collateral venous pathways around the superior sagittal sinus were observed in 3 complete occlusion of sinus lumina.All these manifestations on the images were identical with the findings in operation.Conclusion MRV can display the degree of tumorous erosion to sagittal sinus and the situation of collateral venous pathways,which may be helpful for selecting the surgical approach,making the operation scheme and dealing well with the relationship between tumor and blood vessel in operation.
