吸入一氧化氮对急性大面积肺血栓栓塞家兔血浆TE-1和GMP-140的影响

摘要 目的研究大面积肺血栓栓塞症家兔心肌受损模型的心肌肌钙蛋白(CTnI)浓度、血浆内皮素-1(ET-1)、血浆α-颗粒蛋白浓度(GMP-140)的改变及吸入一氧化氮对其影响,来探讨吸入一氧化氮对大面积肺血栓栓塞症家兔受损心肌保护作用的机理。方法日本大白兔30只,分为试验组10只,只对家兔进行大面积肺血栓栓塞症心肌受损模型实验;对照组10只:在实验组静脉注射血栓栓子时,本组动物模型制作只静脉注射生理盐水,不注射血栓栓子;治疗组10只:大面积肺血栓栓塞症动物模型成功后吸入一氧化氮治疗24小时,一氧化氮吸入浓度为40×10-6g/L。试验组、对照组、治疗组于试验组模型制备成功后每2 h,吸入一氧化氮治疗24 h同步抽血各一次,治疗组吸入一氧化氮治疗同时,试验组、对照组同步吸入50%氧气,多道生理参数分析记录仪,同步检测动脉平均压和肺动脉压。结果家兔大面积肺血栓栓塞症心肌受损动物模型制备成功后24小时后心肌肌钙蛋白浓度明显高于对照组(0.41±0.07,0.08±0.009,P<0.05),吸入一氧化氮24小时后治疗组明显低于实验组(0.31±0.07,0.41±0.07,P<0.05);急性大面积肺血栓栓塞症家兔栓塞后2小时和24小时其血浆内皮素-1水平明显高于对照组(62.20±5.98,46.58±5.25P<0.05;68.09±5.04,45.96±5.30,P<0.05),吸入一氧化氮24小时后其浓度水平明显下降与实验组比较(47.78±5.33,68.09±5.04,P<0.05);家兔大面积肺血栓栓塞症栓塞后2小时和24小时其血浆α-颗粒蛋白浓度水平明显高于对照组(31.04±4.86,16.80±5.16,P<0.05;36.09±4.56,16.72±5.04,P<0.05),吸入一氧化氮24小时后其浓度水平明显下降与实验组比较(18.96±3.59,36.09±4.56,P<0.05);实验组模型制备成功后24小时血浆心肌肌钙蛋白与血浆内皮素-1、血浆α-颗粒蛋白呈明显正相关性(r,0.96,P<0.05;r,0.93,P<0.05);吸入一氧化氮治疗24小时后血浆心肌肌钙蛋白与血浆内皮素-1、血浆α-颗粒蛋白呈明显正相关性(r,0.95,P<0.05;r,0.96,P<0.05)。结论大面积肺血栓栓塞症家兔血浆内皮素-1、血浆α-颗粒蛋白浓度增高是造成心肌受损的主要体液因子,吸入一氧化氮降低其浓度是保护心肌受损的主要机理。 Objective By studying the change of on cardiac troponin I,endothelin-1(ET-1),GMP-140 of rabbits with acute massive pulmonary embolism and these be affacted by inhalation nitric oxide,investigate the reason that is damage cardiac muscle of rabbits with acute massive pulmonary embolism is protected by inhalation nitric oxide.Methods 30 Japanese white rabbits were randomly divided into the experimental group(cute massive pulmonary embolism),contrast group,contral group(onely inject physyologicall salt into the vein),treatment group(cute massive pulmonary embolism rabbits inhalated nitrin oxide of 40×10-6g/l).cardiac troponin concentrationand I,endothelin-1(ET-1),GMP-140 was measured at 2 h for 24 h after animals models has successfully been finished.Results The cardiac troponin I of rabbits with acute massive pulmonary embolism were significantly higher than that control group at 24 h after animals models has successfully been finished(0.41±0.07,0.08±0.009,P0.05),the cardiac troponin I in treatment group were significantly lower than that experimental group after 24 h animals inhalated nitrin oxide(0.31±0.07,0.41±0.07,P0.05);the level of endothelin-1 of rabbits with acute massive pulmonary embolism were significantly higher than that control group after 2h and 24h in the animals models has successfully been finished(62.20±5.98,46.58±5.25 P0.05;68.09±5.04,45.96±5.30,P0.05),the level of endothelin-1 in treatment group were significantly lower than that experimental group after 24 h animals inhalated nitrin oxide(47.78±5.33,68.09±5.04,P0.05),the level of GMP-140 of rabbits with acute massive pulmonary embolism were significantly higher than that control group after 2h and 24 h in the animals models has successfully been finished(31.04±4.86,16.80±5.16,P0.05;36.09±4.56,16.72±5.04,P0.05),the level of GMP-140 in treatment group were significantly lower than that experimental group after 24 h animals inhalated nitrin oxide(18.96±3.59,36.09±4.56,P0.05)the relative was markedly in concentration of cardiac troponin I and the level of endothelin-1 or GMP-140 experimental group after 24 h in the animals models has successfully been finished or treatment group after 24 h animals inhalated nitrin oxide(r,0.96,P0.05;r,0.93,P0.05 or(r,0.95,P0.05;r,0.96,P0.05).Conclusion The level of endothelin-1 or GMP-140 were increased in rabbits with acute massive pulmonary embolism were mainly reason that cardiac muscle were injured,the level of endothelin-1 or GMP-140 were decreased after 24 h inhalation nitrin oxide were mainly reason that cardiac muscle were protected.
