基于无尺度网络方法总结戴希文治疗慢性肾脏病经验

Dai Xiwen's Experience of Herbal Administration in Treating Chronic Kidney Diseases on the Basis of Scale Free Network

目的基于无尺度网络方法进行戴希文教授治疗慢性肾脏病(CKD)用药经验的研究。方法通过将戴老102例CKD患者门诊病历资料录入"临床诊疗信息采集系统",形成戴老临床信息数据库,运用无尺度网络方法进行数据分析,得出处方配伍网络及网络药物关联频度信息,分析结果并与戴老反复交流,得出戴教授治疗CKD的核心方药、随证加减规律,并据此总结戴老临床经验及学术观点。结果总结出核心方药15味,随证加减18组,结果体现出戴老诊治CKD扶正祛邪的总治则,益气活血、清热解毒利湿的主要治法,气血水同治、重视祛邪的临证思路,扶正不壅补的用药特点及以平为期的治疗目标。结论运用无尺度网络方法进行戴老用药经验研究,结果直观、客观,较为深入、完整、如实地反映了戴老治疗CKD的用药经验及部分学术观点,有一定的临床指导意义。

Objective To study Dai Xiwen＇s experience of herbal administration in treating chronic kidney diseases（CKD） on the basis of scale free network.Methods Through inputting the clinical information from Professor Dai＇s 102 cases of CKD to the Clinical Information Collection System,we made the database of Prof Dai＇s clinical information.We obtained the network of herbal compatibility and the frequency information of relative herb medicines with data analysis by using scale free network.After studying the results and communicating with Prof Dai,the core herbal medicines and modification herb groups according to different syndromes in treating CKD were obtained,so as to summarize the clinical experience and academic point of view of Prof Dai.Results Fifteen core herbal medicines and 18 modification herb groups were summarized,indicating that in treating CKD,Prof Dai abide by the general therapeutic principle of strengthening vital qi and dispelling exogenous pathogen,and take benefiting vital qi,activating blood,clearing away the heat evil,expelling superficial evils and eliminating dampness as main therapeutic methods.Meanwhile,she attaches importance to dispelling exogenous pathogen and treating qi,blood and water pathogen at the same time.Her characteristics of herbal administration that promoting vital qi without stagnation and the goal of treatment which is reaching balance were also embodied.Conclusion The results of studying on Dai Xiwen＇s medication experiences by means of scale free network are intuitional and objective.They deeply,integrallty and faithfully reflected Prof Dai＇s experience of herbal administration in treating CKD and revealed part of her academic point of view,thus have positive meaning for clinic.
