右室Tei指数在超声诊断西宁地区肺心病及慢阻肺病中的应用 被引量:4

摘要 目的:利用组织多普勒超声心动图测定西宁地区(海拔2260M)肺心病及慢阻肺病患者右室"Tei指数",探讨Tei指数在评价西宁地区肺心病及慢阻肺病患者右室功能的应用价值。方法:选取本院(西宁地区海拔2260m)2009年7月至2010年9月收治的肺心病患者70例;选择同期在我院收治的慢性阻塞性肺病患者60例,65例来自同期我院体检中心的健康体检人员,采取经胸超声心动图检查,测量右房横径(RA)、右室前后径(RV)、右室前壁厚度(RVAW)及右室Tei指数:在心尖四腔心切面转换为PW-DTI模式,将3.5mm的取样容积置于三尖瓣环右心室游离壁位点,分别测量等容收缩时间(IVCT)、等容舒张时间(IVRT)和射血时间(ET),右室Tei指数=(IVRT+IVCT)/ET。测3个心动周期,取平均值计算出Tei指数。肺心病组有39例经治疗病情缓解后再次检测右心室Tei指数,并与治疗前比较。结果:肺心病组,早期慢阻肺病组和正常对照组超声指标比较:肺心病组右室ICT,右室IRT,右室Tei指数均显著高于正常对照组(P<0.01);早期慢阻肺病组右室IRT,右室Tei指数均明显高于正常对照组(P<0.05,P<0.01);肺心病组治疗前与治疗后右室Tei指数比较:肺心病组中的39例患者治疗后右室Tei指数明显低于治疗前(P<0.05)。结论:对西宁地区肺心病,COPD患者进行超声右室Tei指数监测,可有效评价右室功能,具有很好的临床应用价值。
