注射HCG日血清孕酮水平与IVF结局的关系

目的探讨在控制性超排卵中注射HCG日血清孕酮(P)水平与IVF结局的关系。方法选择广西医科大学第一附属医院生殖中心2006年11月至2008年11月已接受试管婴儿治疗且有完整资料患者共741例,按注射HCG日血清孕酮水平分为≥1.04组与<1.04组以及按雌二醇/孕酮(E2/P)水平分成≥1.51组,<1.51组,分析比较组间各项观察指标的差别。结果不同孕酮组间基础E2,窦泡数(AFC),HCG日≥14mm卵泡数、获卵数、2PN数优胚数、临床妊娠率等差异有统计学意义(P<0.05)。而移植胚胎数、受精率、卵裂率,植入率等的差异均无统计学意义(P>0.05)。结论在控制性超排卵治疗中,HCG注射日血清P值高者临床妊娠率低,E2/P比值是预测助孕结局的重要指标。

Objective To evaluate the impact of progesterone（P） level and E2/P level of HCG administration on outcome sofin vitro fertilization in controlled ovarian hyper stimulation.Methods 741 cycles of IVF/ICSI performed in our reproduction center from November 2006 to November 2008 were analyzed retrospectively.These cycles were stratified according to P levels of ≥1.04 ng/ml or 1.04ng/ml.This cutoff level was selected because its yielded the highest sensitivity and specificity according to a receiver operator characteristic curve.Results In cycles with high serum p levels,less oocytes were retrieved,clinical pregnancy rate was 33.8%（in cycles with low serum P level were 44.6%）,respectively（P〈0.05）.Conclusion Our data showed adverse effect of high serum P levels on the day of HCG administration on clinical pregnancy rate.
