在剖宫产术中行子宫肌瘤剜除术的临床研究

Clinical Study of Myomectomy During Cesarean Section

【目的】探讨剖宫产术中行子宫肌瘤剜除术的可行性和安全性。【方法】回顾性分析40例妊娠合并子宫肌瘤孕妇的临床资料，选择同期无任何并发症的正常剖宫产患者80例为对照组，比较两组术中平均出血量、手术时间、住院天数及产后出血率、产褥病发生率。【结果】两组术中平均出血量、手术时间、住院天数、产褥病发病率比较无显著性差异（P〉o．05）；35例手术患者未出现肌瘤复发，有2例患者出现月经量过多，5例未处理肌瘤患者中有1例出现月经量过多，对照组月经多为正常。【结论】在做好充分的术前准备，把握好手术指征的情况下，在剖宫产术中行子宫肌瘤剜除术是可行的。

[Objective] To explore the safety and feasibility of myomectomy during cesarean section. [Methods] The clinical data of 40 pregnant women with hysteromyoma were analyzed retrospectively. Eighty normal pregnant women without complications after cesarean sections were enrolled as the control group. The average bleeding volume, operation time, hospitalization day, postpartum hemorrhage and incidence of post- partum infection were compared between two groups. [Results] There was no difference in the average blood loss, operation time, hospitalization day and incidence of postpartum infection between two groups（ P 〉0. 05）. No recurrence of hysteromyoma was found in 35 surgical patients. The excessive menstrual blood volume was found in 2 cases and 1 case of 5 untreated patients. The menstruation of the control group was normal. [Conclusion] If the preoperative preparation is sufficient and the surgical indications are handled, myomectomy during cesarean section is feasible.
