乌司他丁治疗弥漫性血管内凝血的疗效分析

Treatment effects of Ulinastatin on Disseminated Intravascular Coagulation

目的评价乌司他丁对弥漫性血管内凝血(DIC)的干预效果。方法将DIC患者随机分为3组:对照组、20万U组和40万U组,3组病例分别静注生理盐水、20万U和40万U乌司他丁,治疗前后监测患者的凝血指标及血小板数。结果治疗后第3天与治疗前比较,20万U组和40万U组患者的PT或APTT都获得改善;治疗后第5天,凝血指标和血小板数均明显改善;3组患者互相比较,20万U组和40万U组患者的四项凝血指标及血小板比对照组均有明显改善,20万U组和40万U组患者比较,40万U组患者的PT改善更显著,其他指标差异无统计学意义。结论乌司他丁对DIC患者具有积极的干预作用,高剂量可能效果更好。

Objective To study the treatment effects of ulinastatin （UTI） on disseminated intravascular coagulation （DIC）. Methods DIC patients were randomly divided into three groups： control group,200 kU UTI group and 400 kU UTI group. The three group patients were administered NS,200 kU UTI and 400 kU UTI respectively. Clinical parameters were determined by evaluation with PT,APTT,FIB,D-D and PLT. Results 3 days after admission,compared to before treatment,the PT and APTT were improved significantly in three groups. 5 days after admission, that made more improvement in PT, APTr, FIB, D-D and PLT. Compared to control group, PT, APTT, FIB, D-D and PLT were increased significantly within 200 kU UTI and 400 kU UTI groups. Compared to 200 kU UTI group,PT was increased significantly within 400 kU UTI group, and there were not significantly different within other parameters. Conclusion Ulinastatin had a beneficial role in the treatment of DIC, high dose UTI had more effects possibly.
