中西医结合治疗肝胃郁热型慢性胃炎临床研究

Clinical Study on Integrated Traditional Chinese and Western Medicine in the Treatment of Chronic Gastritis due to Accumulation of Heat in the Liver and Stomach

目的:观察三参胃炎汤联合西药治疗肝胃郁热型慢性糜烂性胃炎的临床疗效。方法:100例符合纳入标准的慢性糜烂性胃炎患者,根据就诊先后顺序随机(1∶1)分为两组各50例。对照组采用:奥美拉唑胶囊20 mg,2次/d,餐前服;阿莫西林胶囊1.0 g,2次/d;克拉霉素片0.5 g,2次/d,餐后服。7 d后服奥美拉唑胶囊20 mg,1次/d,再服用14d。治疗组在对照组基础上加用三参胃炎汤,1付/d,水煎分早、晚2次温服。两组均21 d为1疗程,治疗期间禁吃生冷寒凉辛辣厚味食品。结果:①两组有效率:治疗组为88.00%,对照组为72.00%,经χ2检验,有显著性差异(P<0.05),治疗组优于对照组。②两组抗HP总有效率:治疗组为86.00%,对照组为64.00%,经χ2检验差异有统计学意义(P<0.05)。结论:三参胃炎汤联合西药治疗肝胃郁热型慢性糜烂性胃炎有显著的临床疗效,对幽门螺杆菌有极好的清除作用且副作用少,依从性好,值得临床进一步研究。

Objective：To observe the clinical effects of Sanshen Weiyan Decoction integrated with Western Medicine in treating chronic erosive gastritis due to accumulation of heat in the liver and stomach.Methods：100 patients of chronic erosive gastritis in accordance with inclusive criteria were randomly divided into two groups,each of 50 cases,according to the orders of their treatment.Patients in control group were treated by Omeprazole Capsules,20mg each time and twice a day before meals,and Amoxycillin Capsules were taken by 1.0g each time and twice a day;after meals,Clarithromycin Tablets 0.5g were taken each time and twice a day.Seven days later,Omeprazole Capsules were taken 20mg each time and once a day,for another 14days.Patients in treatment group were treated by Sanshen Weiyan Decoction besides the drugs taken by the control group,one formula each day,after fried with water,and the decoction should be taken twice,once in the morning and once in the evening.Both groups took 21days as a course of treatment,during which patients were forbidden to have cool,cold,bitter and feast foods.Results：① Total effect comparisons between the two groups showed that the mean rank of the control group was 43.54,while the treatment group was 57.46.The Mann-Whitney U statistics were 902.00;Wilcoxon were 2177.00;the Z test statistics Z=2.594,p=0.009（double-sided）,and the effects of the two groups had significant difference,which showed that the treatment group was better than the control group.② The total effective rates of the two groups showed that the treatment group was 88.00% while the control group was 72.00%.After being compared,x2 =4.000 v=1 and p=0.046,which showed that the difference between both groups was significant and the treatment group was better than the control group.③ The total effective rates of the two groups on fighting high pressure（HP）：the treatment group was 86.00% while the control group was 64.00%,and after being compared,x2 =6.453 v=1,p=0.011.Conclusion：Sanshen Weiyan Decoction integrated with Western Medicine has significant clinical effects on chronic erosive gastritis due to accumulation of heat in the liver and stomach,and it has good clearing effect on Helicobacter pylori and Hp with little side effects,which has favorable compliance and is worth being furtherly studied in clinic.
