脑出血微创术抽吸力量与术后再出血关系研究 被引量:3

Relationship between Power of Aspiration and Incidence of Early Rebleeding of Mini-traumatized Transcranial Puncture

摘要 目的探讨抽吸力量大小与微创颅内血肿穿刺清除术治疗高血压脑出血后早期再出血的发生率的关系,探索微创颅内血肿清除术的最佳抽吸力量。方法 160例高血压脑出血患者随机分为两组,均予以颅内血肿微创穿刺清除术,治疗组80例,抽吸力<2 N,对照组80例,抽吸力>3 N。对两组初次手术治疗后1 d内再出血发生率及预后进行评估,并进行统计分析。结果治疗组发生再出血8例,发生率10%,经积极治疗后死亡4例,死亡率5%。对照组发生再出血17例,发生率21.2%,经积极治疗后死亡10例,死亡率12.5%。治疗组术后早期再出血率明显低于对照组,其差异有显著性(P<0.05)。并且治疗组死亡率低于对照组,其差异也有显著性(P<0.05)。结论微创清除术中抽出血肿时抽吸力<2 N,再出血发生率低,并明显降低了脑出血患者的死亡率。 Objective To investigate the relationship between power of aspiration and incidence of early rebleeding of mini-traumatized transcranial puncture to patients with hypertensive intracerebral hemorrhage,and to explore the best aspiration power of mini-traumatized transcranial puncture. Methods 160 cases of hypertensive cerebral hemorrhage were randomly divided into two groups.All patients underwent mini-traumatized transcranial puncture of intracranial hematoma,aspiration power of the treatment group 2 Newton,aspiration power of the control group 3 Newton.To assess the rebleeding incidence and prognosis of the two groups after the initial surgery within 1 day,and to analysie the data. Results 8 cases of rebleeding in treatment group,the incidence is 10%,4 patients died after active treatment,the mortality rate was 5%.17 cases of rebleeding in control group,the incidence was 21.2%,10 atients died after activetreatment,the mortality rate was 12.5%.Early rebleeding after the treatment group was significantly lower than the control group,the difference was significant(P0.05).And mortality in the treatment group was lower than control group,the difference was also significant(P0.05). Conclusion Aspiration power 2 Newton,in mini-traumatized transcranial puncture reduce the incidence of recurrent bleeding and significantly reduce mortality in patients with cerebral hemorrhage.
