摘要
目的探讨冠状动脉粥样硬化性心脏病(冠心病)患者血浆8-表氧-前列腺素F2α(8-iso-prosta-glandin F2α,8-iso-PGF2α)水平、胰岛素抵抗状况及两者在冠心病发展过程中的意义。方法 130例冠心病患者分为稳定型心绞痛组(n=43)、不稳定型心绞痛组(n=41)、急性心肌梗死组(n=46),对照组为非冠心病患者(n=68)。应用酶联免疫双抗体夹心法测定血浆8-iso-PGF2α水平,按照稳态模型胰岛素抵抗指数(HOMA-IR)=FPG×FINS/22.5公式计算胰岛素抵抗状况。并将冠心病组上述指标与冠状动脉(冠脉)病变支数Gensini积分进行分析。结果与对照组比较,冠心病各组血浆8-iso-PGF2α、HOMA-IR水平显著升高(P<0.05);并随着冠脉受累支数增多,血浆8-iso-PGF2α、HOMA-IR水平逐渐升高(P<0.05)。结论冠心病患者体内存在氧化应激及胰岛素抵抗,血浆8-iso-PGF2α、HOMA-IR的水平对判断冠心病严重程度有一定意义。
Objective To investigate the level of plasma 8-iso-prostaglandin F2α and insulin resistance in CHD patients,and their significance in the development of CHD.Methods All 130 CHD patients were divided into the group of stable angina pectoris(SAP group,n=43),group of unstable angina pectoris(UAP group,n=41) and group of acute myocardial infarction(AMI group,n=46).The control group included the patients of non-CHD(n=68).The level of plasma 8-iso-prostaglandin F2α was detected by using enzyme-linked immunosorbent assay(ELISA) and the insulin resistance was calculated according to the formula of(HOMA-IR)=FPG×FINS/22.5.All above indexes and Gensini integrals of diseased coronary vessels were analyzed in CHD patients.Results Compared with the control group,the levels of plasma 8-iso-prostaglandin F2α and HOMA-IR increased significantly in other groups(P0.05),and would rise gradually along with the increase of disease coronary vessels and aggravation of CHD(P0.05).Conclusion Exist in vivo oxidative stress in patients with coronary heart disease and insulin resistance,plasma 8-iso-PGF 2α,HOMA-IR to determine the level of severity of coronary heart disease have a certain significance.
