摘要
目的:探讨胸腔积液中脑钠肽(BNP)和血清腺苷脱氨酶(ADA)的水平对鉴别诊断结核性、恶性胸腔积液的临床价值。方法:选择结核性胸腔积液患者26例、恶性胸腔积液患者30例,分别取患者胸腔积液5 ml送检,采用ELISA法测定胸腔积液中BNP水平,用比色分析法检测胸腔积液ADA含量,并对测定结果进行统计学分析。结果:恶性胸腔积液中BNP水平明显高于结核性组,ADA水平明显低于结核性组,两组比较差异有统计学意义(P<0.01)。BNP与ADA在同一性质胸腔积液中的检测结果呈负相关性。结论:胸腔积液中BNP、ADA水平的测定有助于结核性及恶性胸腔积液的鉴别诊断,两者联合进行诊断的意义有待于进一步研究。
Objective: To evaluate the clinical value of brain natriuretic peptide(BNP) and serum adenosine deaminase(ADA) levels of pleural effusion in the differential diagnosis for tuberculotic or malignant pleural effusion.Methods: 26 patients with tuberculotic pleural effusion and 30 patients with malignant pleural effusion were enrolled into the study,and 5 ml pleural effusions were collected from each patient for Lab.test.ELISA and colorimetric assay were respectively used to determine the BNP levels of pleural effusion and contents of ADA in pleural effusion,and the determination results were statistically analyzed.Results: BNP levels in malignant pleural effusion was significantly higher than those in tuberculotic pleural effusion,ADA levels were significantly lower in malignant pleural effusion than in tuberculotic pleural effusion,the differences were statistically significant(P0.01);the critical value of BNP for the determination of malignant pleural effusion was 466.50 gp / ml and the critical value of ADA for determination of tuberculotic pleural effusion was 45 U / L.The test results in the determination for same pleural effusion were negatively correlated between BNP and ADA.Conclusion: The determination of BNP and ADA in pleural effusion is helpful for the differential diagnosis of tuberculotic or malignant pleural effusion and the significance of joint diagnosis remains to be studied further.
