摘要
目的:观察运动是否改变2型糖尿病患者血清瘦素和脂联素水平,分析这两种细胞因子及其交互作用的变化与运动改善胰岛素抵抗之间的关系。方法:21名2型糖尿病妇女随机分为运动组(n=11)和对照组(n=10),进行24周,每周3~4次的有氧结合抗阻力运动干预。实验前、后检测肥胖相关指标、血脂、血糖、胰岛素以及血清细胞因子脂联素和瘦素水平,计算HOMA-IR。结果:与对照组相比,运动组胰岛素敏感性显著升高,肥胖相关指标下降,HDL-C升高;瘦素水平下降,脂联素水平升高,细胞因子的变化是胰岛素敏感性改善的最佳预测因素。广义线性模型(GEE)分析显示,瘦素降低了胰岛素的敏感性,但在实验后运动组二者的交互作用下降。结论:运动改善了2型糖尿病妇女胰岛素的敏感性,降低了瘦素水平,升高脂联素水平。推测,对于人类2型糖尿病患者,瘦素也会抑制脂联素发挥胰岛素的增敏作用。
Objective:To observe the change of serum adiponectin and leptin of type 2 diabetes caused by exercise,analyze the relation between its interaction and exercise-induced insulin resistance improvement.Methods:21 female type 2 diabetes are randomly divided into exercise group(n=11) and control group(n=10),both group engaged in a 24-week program of 3~4 hours per week aerobic exercise plus resistance training,versus no extra physical activity.Obesity-related variables were measured,such as lipids,leptin,adiponectin,insulin,and glucose.HOMA insulin resistance was calculated.Results:Compared to control group,insulin sensitivity increased in the experimental group.Obesity decreased,and HDL cholesterol increased.Changes in adipokines were the best predictor of the increase of insulin sensitivity.Lower leptin coincided with increased insulin sensitivity.At baseline,high standardized leptin adiponectin related to more insulin resistance,but at post-test,this was not so,or hardly so,with lower leptin,and higher adiponectin that coincided with lower insulin resistance.It is concluded that exercise led to a decrease of leptin,and an increase of adiponectin and insulin sensitivity.It suggested that high leptin inhibited adiponectin to have its insulin-sensitizing effects on subjects with type 2 diabetes.
