超声诊断早孕期妇女卵巢的形态变化

摘要 目的:探讨超声监测早孕妇女卵巢形态变化的临床应用价值,以提高对早孕期卵巢形态的认知。方法:对130例以往诊断的早孕妇女卵巢形态进行回顾性分析,130例早孕妇女均为自然怀孕、孕前无子宫附件疾患且早孕时间为5~11周,门诊常规早孕超声检查,探测卵巢并观察其形态变化及血流特点。结果:130例早孕妇女中,一侧卵巢中有不均质低回声或偏等回声考虑为黄体的共59例,56例为一侧卵巢内以囊性为主的不均质回声,内或可见形态不规则的高回声团块及丝网状不均质回声。一侧卵巢单纯囊肿考虑黄体囊肿的13例,表现为无回声、壁薄、内透声佳。卵巢中仅见囊样卵泡者2例。结论:早孕时,卵巢形态发生一系列相应变化,以妊娠黄体及黄体囊肿存在为主。 Objective:To evaluate the diagnostic value of two dimension ultrasound(2DUS) and color Doppler flow imaging(CDFI) for ovarian morphologic changes of early pregnancy and to improve prognosis of early pregnancy.Methods:The sonographic echogenicity of ovarian morphologic changes and blood supply distribution in 130 women with early pregnancy was retrospectively reviewed.These pregnant women were natural pregnancy without adnexal disease whose duration of pregnancy was 5-11 weeks.Results:Among 130 early pregnant women,the echogenicity of 59 cases was nonhomogeneous hypoecho or iso-echo of corpora luteum in unilateral ovary;56 cases was nonhomogeneous hypoecho of cystic in unilateral ovary with irregular heperecho mass and webby heterogeneous echo;Cyst in unilateral ovary of 13 cases was consider as luteumcyst with anecho,wall thinning,good entrant sound;only 2 cases was observed cystic ovarian follicle.Conclusion:Ovarian morphologic changes of early pregnancy would be mainly corpora luteum graviditatis and luteumcyst.
