自拟中药骨伤Ⅰ号方对踝关节骨折术后肿痛及功能改善的疗效观察

Clinical observation of traumatology&orthopedicsI in the treatment of ankle fracture postoperative pain,swelling and functional rehabilitation

目的:研究并探讨自拟中药骨伤Ⅰ号方对踝关节骨折术后肿痛症状及功能康复改善的治疗效果。方法:采取前瞻性随机对照试验的研究方法,选取2007年11月到2009年9月之间96例满足条件的踝关节骨折手术治疗病例作为研究对象。术后两组均给予塞来昔布片口服镇痛,分为治疗组(A组)服用自拟中药骨伤方,对照组(B组)口服安慰剂,疗程为术后1周。术后对患者进行随访,内容包括肿胀值评定、VAS疼痛评分、止痛效果分析及术后6月的Baird-Jackson、AOFAS评分等,最后进行统计学分析。结果:在术后不同时间点对比,治疗组VAS均值比对照组低,但两组差异无统计学意义(P>0.05)。在肿胀值比较上,治疗组在术后3天、7天的肿胀值较对照组稍低,差异有统计学意义。术后Baird-Jackson及AOFAS评分中,治疗组评分均高于对照组,差异有统计学意义(P<0.05)。结论:自拟中药骨伤Ⅰ号方能较快改善踝关节骨折术后肿胀、疼痛,促进关节功能的恢复。

Objective： To observe the clinical efficacy of traumatology＆orthopedicsI in the treatment of orthopaedic postoperative pain, swelling and functional rehabilitation. Methods： In the prospective randomized controlled study, 96 postoperative ankle fracture cases from November of 2007 to September of 2009 who needed to receive oral analgesic were divided into the treatment group and the control group. The treatment group was given traumatology＆orthopedicsI and the control group was given the placebo. The therapeutic period was 7 days. The functional recovery was evaluated in terms of the engorgement score, VAS score, Baird - Jackson score, and AOFAS score, all of which were then statistically analyzed. Results ： The VAS score in the treatment group is lower than the one in the control group. Besides, there was obvious variance in engorgement relief score in the 3th, 7th postoperative day. The Baird - Jackson and AOFAS score in the treatment group which had obvious variance compared with the control group （ P 〈 0. 05）. Conclusion： The traumatology＆orthopedicsI can improve the ankle fracture postoperative pain, swelling and functional rehabilitation rapidly.
