胆囊癌CT诊断与鉴别诊断

CT diagnosis of Gallbladder Carcinoma

目的探讨各型胆囊癌CT征象,以提高胆囊癌诊断水平和准确率。方法回顾性分析经病理证实且资料较完整23例胆囊癌CT表现。结果胆囊癌CT表现:胆囊壁增厚型7例,腔内结节型6例,肿块型10例。合并肝脏、其他脏器受侵犯,肝转移,淋巴结转移,肝内外胆管扩张,胆结石等间接征象。结论 CT在胆囊癌的诊断与鉴别诊断中具有重要作用,但存在一定局限性。仔细分析胆囊及周围结构、脏器改变,有助于提高胆囊癌的诊断准确率。

Objective To investigate the CT findings of gallbladder carcinoma and to elevate its diagnostic level and correct.Methods CT findings in 23 cases with pathologically proved gallbladder carcinoma were retrospectively analyzed.Results The major CT findings were as follows：gallbladder wall thickening type in 7,intra-cystic nodule type in 6 and the mass in 10 cases. Other CT findings included direct invasion of liver and other adjacent organs ,hematogenous liver matastases, metastatic lymphadenopathy, both intraand extra-hepatic biliary dilatation, gallstone.Conclusion CT plays an important role in the diagnosis and differential diagnosis of gallbladder carcinoma, but it is still limited.It is helpful to study carefully the changes of the gallbladder and its surrounding structure and organs for the correct diagnosis of gallbladder carcinoma on CT.
