摘要
目的探讨免疫功能正常人原发性颅内B细胞淋巴瘤常规MRI及多体素质子磁共振波谱成像(1H-MRS)的表现。方法回顾性分析11例经病理证实的原发性颅内B细胞淋巴瘤的常规MRI和1H-MRS表现。结果 11例患者共检出16个病灶,病灶T1WI呈稍低信号,T2WI呈等或稍高信号,单发或多发,瘤周多呈轻中度水肿,增强后病灶多明显均匀强化,"尖角征"和"缺口征"的出现具有特异性。1H-MRS表现为胆碱(Cho)峰升高,氮-乙酰天门冬氨酸(NAA)峰降低,并出现高耸的脂质(Lip)峰。结论原发性颅内B细胞淋巴瘤MRI、1H-MRS具有一定的特征,联合应用可提高MRI对肿瘤的诊断水平。
Objective To retrospectively analyze conventional MR imaging and 1H-MR spectroscopy(1 H-MRS) features of primary intracranial lymphoma in patients with normal immunological function. Methods The MRI and 1H-MRS features were retrospectively analyzed in 11 cases of B-cell primary intracranial lymphoma proved by operation and pathology,while 1H-MRS features of 9 cases were retrospectively studied. Results 16 nodules were detected in all 11 patients,The lesions showed isointensity on TlWI,isointensity or slightly hyperintensity on T2WI,and had mild or moderate edema. Most lesions demonstrated"gap sign","cusp sign"and obvious Enhancement was intense and homogenous,the presence of "incision sign" and "angular sign" were more specific. 1H-MRS showed decreased NAA and elevated choline(Cho) with large lipid peak. Conclusion Combining with the conventional MRI appearance with 1H-MRS features can improve MRI diagnostic accuracy of primary intracranial lymphoma.
