中西医结合治疗过敏性紫癜100例临床观察

Observation of henoch-schonlein purpura treated by therapy of integrated traditional and western medicine

目的观察中西医结合治疗过敏性紫癜(HSP)的临床疗效。方法将200例HSP患者随机分为3组。中药组45例予中药煎剂治疗,西药组55例予西药治疗,中西医结合组100例予中西药结合治疗。3组均7 d为1个疗程,治疗3个疗程后评价疗效。结果中西医结合组痊愈率均高于中药组和西药组(P<0.05)。中药组、西药组和中西医结合组血小板平均容积(MPV)、血小板分布宽度(PDW)及血小板压积(PCT)均较本组治疗前降低(P<0.05),MPV、PDW、PCT可作为临床疗效评价的参考。3组治疗前后凝血酶原时间(PT)、凝血酶时间(TT)、活化部分凝血活酶时间(KPTT)比较差异无统计学意义(P>0.05)。结论 MPV、PDW、PCT均增加,与HSP发病呈正相关,提示血小板可能参与HSP发病过程。中西医结合治疗HSP,均符合HSP病机,二者联合应用,既增强了治疗作用,又减少了西药不良反应,取得良好疗效。

Objective To observe the effect of henoch-schonlein purpura（HSP） treated by therapy of integrated traditional and western medicine.Methods 200 patients with HSP were randomly divided into three groups.Chinese medicine group（n=45） was given Chinese medicine decoction.Control group（n=55） was given routine western medicine.100 cases in integrative Medicine with Western Medicine group were given therapy of integrated traditional and western medicine.The therapeutic course was seven days in three groups.The effect was evaluated after three courses.Results The mean platelet volume and platelet distribution width after treatment was decreased as compared with those before treatment in three groups（P0.05）.Platelet hematocrit was increased as compared with that before treatment（P0.05）.Mean platelet volume,platelet distribution width,platelet hematocrit can be used as reference for clinical evaluation.There was no significant difference between before and after treatment on prothrombin time（PT）,thrombin time（TT） and activated partial thromboplastin time（KPTT） in 3 groups（P0.05）.Conclusion The increase of mean platelet volume,platelet distribution width,platelet hematocrit was positively correlated with the incidence of the HSP.Platelets may be involved in the pathogenesis of HSP.Treating HSP by therapy of integrated traditional and western medicine is in line with HSP pathogenesis.The therapy of integrated traditional and western medicine Not only enhanced the therapeutic effect,and reduced side effects medicine.
