高频振荡通气联合常频通气治疗新生儿呼吸衰竭的临床观察 被引量:4

摘要 目的探讨高频振荡通气(HFOV)加常频机械通气(CMV)对新生儿呼吸衰竭的治疗作用。方法 17例应用CMV治疗失败的呼吸衰竭新生儿改用HFOV加CMV(HFOV+CMV)治疗,比较治疗前后心率、平均动脉压、PO2、PCO2及氧合指数(OI)。结果 HFOV+CMV治疗后1h、6~12h心率、平均动脉压与治疗前比较差异无显著性(P〉0.05),HFOV+CMV治疗后1h、6~12h,PO2均升高、PCO2、OI均下降,与治疗前比较差异有显著性(P〈0.01)。结论对于CMV治疗失败的新生儿呼吸衰竭,改用HFOV+CMV后氧合及通气功能可得到改善,且对患儿的心血管系统影响不大,有一定疗效。 Objective The purpose of this study was to probe the high frequency oscillatory ventilation(HFOV) to add convention machinery ventilation(CMV) to the newborn respiratory failure therapeutic action.Methods 17 examples newborn were treated using(HFOV+CMV) with CMV treatment defeat's respiratory failure,compared with treatment around heart rate,average arterial pressure,P02,PC02 and oxygenate index(OI).Results The heart rate and average arterial pressure after HFOV+CMV treatment 1 hour,6-12 hour,comparison difference non-significance(P0.05);the P02 was elevated,PC02 and OI was decreased after HFOV+CMV treatment 1 hour,6-12 hour,so a significant difference existed between them(P0.01).Conclusion The oxygenate and the ventilation function may be improved using HFOV+CMV treatment for the CMV treatment defeat's newborn respiratory failure,and do not effect cardiovascular system.
