血常规报告异常细胞漏检和少检分析

Analysis of missed and low abnormal cells detection in blood routines

目的探讨血常规报告异常细胞的漏检或少检情况,提高白细胞分类结果的准确性。方法比较2 498例血常规白细胞分类和当天患者作骨髓检查时检查外周血涂片的结果,对异常细胞检出率的差异及细胞类型进行统计分析。结果分类结果以骨髓细胞室血涂片为准,门诊血常规异常细胞漏检标本360例,占14.41%,其中异常细胞≤5%标本209例,异常细胞6%～10%标本88例,异常细胞>10%标本63例;在2实验室均检出异常细胞的223例标本中,血常规分类百分比低于骨髓细胞室标本112例,占4.48%,2实验室分类百分比分别为22%和36%,差异具有统计学意义(P<0.01)。结论血常规报告异常细胞的漏检时有发生,报告的审发应严格遵循临床检验中心的镜检复片标准。

Objective To study abnormal cells of missed and low detection in blood routine,in order to improve the accuracy of leukocyte classification.Method Compared with 2 498 leukocyte classification in blood and bone marrow smears of collecting sample on the same day,statistical analysis including abnormal cells detection,the rate difference and cell types were done.Results Three hundreds and sixty cases of abnormal cells were undetected in blood routine room,according to the result of bone marrow cell room,the missed rate was 14.41%.Of the abnormal cells percentage,209 cases were ≤5%,88 cases were 6%～10%,63 cases were 10%.There were 223 cases detected abnormal cells in the two groups,but 112 cases detected the percentage of abnormal cells in blood routine room were lower than in bone marrow room,the rate was 4.48%.The average value of the abnormal cells percentage in the two groups were 22% and 36% respectively and their difference had statistically significance（P0.01）.Conclusion Abnormal cells miss frequently occurs at blood routine examination,the blood routine report must strictly follow the standard of the clinical test center by microscopic examination.
