急性混合细胞白血病伴BCR/ABL融合基因阳性的临床分析

Clinical analysis of mixed phenotype acute leukemia with BCR/ABL fusion gene positive

目的分析伴BCR/ABL融合基因阳性的急性混合细胞白血病(MPAL)临床特点、生物学特性、临床疗效及预后。方法对1例伴BCR/ABL融合基因阳性的MPAL患者临床及实验室资料进行分析,结合细胞形态学、免疫分型及融合基因定性检测进行诊断,并予以多次不同化疗方案治疗。结果患者在治疗期间反复出现严重肺部感染,最终死于多脏器衰竭。结论 MPAL病例罕见,且无统一的治疗方案,预后差。年龄和Ph染色体阳性与患者预后紧密相关。

Objective Objective To analyze the clinica1 and biological features,as well as the effect of treatment and prognosis of mixed phenotype acute leukemia（MPAL）.Methods To analyze the clinical and laboratory data of a MPAL patient with BCR/ABL fusion gene positive,diagnose it with cytomorphology,immunophenotype and fusion gene,and treat the patient with multiple different chemotherapies.Results The patient suffered from severe lung infection during treatment and finally died of multiple organ failure.Conclusion The incidence of MPAL is very low,and there are no agreed treatment protocols for patients.It carries a poor prognosis which is most strongly correlated to age and Philadelphia chromosome positivity.
