门诊不合理处方分析

Analysis of outpatient′s irrational prescriptions

目的规范处方书写,提高处方质量,提供更优质的药学服务。方法采用回顾性统计分析方法,对本中心儿童医院院区门诊2008年7月—2009年11月的不合理处方进行统计分析。结果 2008年7月—2009年11月间,共有11 961张不合理处方,约占总处方量的0.5%。其中用药不适宜处方6 397张,占总不合理处方的53.48%,用药不适宜表现为重复用药、联合用药不当、用法用量错误等;其余的不合理处方主要为医师漏签名、规格书写错误或需皮试的药物漏写是否进行皮试等。结论医生和药师应有更多的沟通,共同努力规范处方书写,提高合理用药水平。

Objective To evaluate the irrational prescriptions in recent years for standardizing priscription behavior and for providing better pharmaceutical care.Methods We analyzed the irrational prescriptions in out-patient departments of Guangzhou Children＇s Hospital from July 2008 to November 2009 with retrospective analysis method. Results 11,961 irrational prescriptions,accounting for 0.5% of total prescriptions,were anlyzed from July 2008 to November 2009,including 6,397 improper medical use prescriptions,which was accounting for 53.48% of total irrational prescriptions.Improper medical use were as follows： repeat medication,inappropriate drug combination,dosage errors,etc.Without physicians＇ signing,wrong written specification or without skin test mark were the main other irrational prescription behaviors. Conclusion Although the percentage of irrational prescriptions was relatively small,but doctors and pharmacists should work together to lead to high qualified prescriptions for rational administration.
