皮肌炎77例临床分析

Clinical analysis of 77 patients with dermatomyositis

回顾分析77例皮肌炎病例 ,男女比在21～60岁间为1∶1.9 ,在≤20岁和>60岁为2∶1。初发症状表现皮炎者51.9 % ,皮炎肌炎同时发生占26 %。18.2 %出现间质性肺炎 ,10.4 %出现胸腔积液。EB病毒VCA/IgA抗体几何均数 :皮肌炎并鼻咽癌为1∶19.5、皮肌炎为1∶3.7。肌电图87 %示肌源性损害 ,肌肉活检81 %诊断肌炎。14例合并恶性肿瘤 ,其中鼻咽癌9例。

Seventy seven patients with dermatomyositis(DM) were retrospectively analyzed. The ratio of male to female was 1∶1.9 in patients aged 21～60 years old, 2∶1in patients aged ≤20and >60 years old. 51.9% of cases showed dermatitis as initial psesentation and 26% of cases showed dermatitis and myositis simutaneously. 18.2% of cases showed interstitial pneumonia and 10.4% of cases showed pleural effusion. The geometrical average of VCA/IgA antibody to EB virus was 1∶19.5 in DM accompanied with nasopharyngeal carcinoma(NPC), and 1∶3.7 in DM. Electromyogram and muscle biopsy revealed myogenic damage (87%) and myositis(81%) respectively, 14 cases were associated with malignancies, in which 9 cases of NPC.
