糖调节受损对急性冠脉综合征患者Gensini评分的影响

摘要 目的探讨糖调节受损(IGR)的急性冠脉综合征(ACS)患者冠状动脉病变特点及侧支循环(CC)情况。方法确诊为ACS的166例患者按糖耐量试验(OGTT)分成糖尿病(DM)组56例、糖调节受损(IGR)组66例及血糖正常(NGT)组44名。结果 IGR组及DM组较NGT组更容易发生多支血管病变(P<0.05)和严重狭窄病变(P<0.05),但侧支循环建立无明显差别(P>0.05)。结论早在IGR期冠状动脉就可以有动脉粥样硬化病变,需要尽早加以防治。 Objective To approach the effect of impaired glucose regulation on Gensini score and coronary lesion severity in ACS patients. Methods 166 consecutive patients with confirmed CAD (coronary artery disease) from August 2006 to Oct 2008 underwent oral glucose tolerance test (OGTT). 56 cases met the criteria for diabetes mellitus (DM), 66 for impaired glucose regulation (IGR) and another 46 patients for normal glucose tolerance (NGT). Results Compared to normal glucose patients, patients with diabetes or impaired glucose regulation were more likely to develop multivessel disease (P〈0. 05) and severe stenosis lesions (P〈0. 05). However, coronary collaterals in patients with DM or IGR (P〉0. 05) were similar to normal glucose patient. Conclusion Coronary atherosclerosis may occur during the IGR stage, indicating IGR and DM are the same hazardous factor. Something should be done as soon as possible to prevent the damage by IGR.
