闭合性腹部损伤121例小肠破裂的诊治体会

Experience of diagnosis and treatment of small intestinal rupture in 121 cases of closed abdominal injury

目的:总结闭合性腹部损伤小肠破裂的诊断及治疗。方法:对我院自2002年6月～2009年4月收治121例闭合性腹部损伤小肠破裂的临床资料进行总结分析。结果:本组121例中急性腹膜炎89例,占74%;膈下游离气体12例,占10%;腹穿及腹腔灌洗阳性16例,占13%;剖腹探查术中确诊4例,占3%。均进行外科手术治疗。结论:闭合性腹部损伤小肠破裂的早期诊断,细致的外科手术是治疗和预后的关键。

Objective：To summarize the diagnosis and treatment of small intestinal rupture in closed abdominal injury.Methods：The clinical data in 121 cases of closed abdominal injury complicating small intestine rupture in our hospital from June 2002 to April 2009 were summarized and analyzed.Results：Among 121 cases,89 cases were acute peritonitis,accounting for 74% and 12 cases were subphrenic free air,accounting for 10%.Abdominocentesis and positive peritoneal lavage were in 16 cases,accounting for 13%.In laparotomy,4 cases were diagnosed,accounting for 3%,and underwent surgery.Conclusion：The early diagnosis of small intestine rupture in closed abdominal injury and meticulous surgery are the keys to the treatment and prognosis.
