妇科腹腔镜手术的临床分析

Clinical analysis of laparoscopic surgery in gynecology

目的探讨腹腔镜手术治疗妇科疾病的临床应用价值。方法回顾分析2000年及2009年行腹腔镜手术的652例病例,比较手术率、疾病种类、手术类型、并发症、手术时间、术中出血、术后最高体温及住院天数的变化。结果 B组腹腔镜手术率27.62%,A组腹腔镜手术率8.08%,B组腹腔镜手术率明显升高,差异有统计学意义(P<0.05);B组疾病种类、手术类型较A组增多,B组手术时间较A组缩短(P<0.05),B组术后最高体温较A组低(P<0.05),两组术中出血与术后住院天数差异无统计学意义(P>0.05);中转开腹率B组0.85%较A组的4.55%明显下降(P<0.05),并发症率B组0.51%较A组的10.6%明显下降(P<0.05)。结论腹腔镜手术适应证拓宽,是妇科手术发展的方向。

Objective To evaluate the value of laparoscopic surgery in the treatment of gynecologic diseases.Methods 652 cases of laparoscopic surgery were retrospectively analyzed between 2000 and 2009.The laparoscopic surgery rate,the kinds of diseases,types of surgical,complications,operating time,the amount of bleeding,the highest body temperature of postoperative,hospital stay were analyzed.Results The laparoscopic surgery rate of group B（27.62%） showed significant increases compared with group A（8.08%）（P〈0.05）;The group B showed increases at the kinds of diseases and type of surgical compared with group A;Operation time of group B was shorter（P〈0.05）,the highest body temperature of postoperative was dropping（P〈0.05） compared with group A;The conversion to laparotomy rate of group B（0.85%） showed significant decrease compared with group A（4.55%,P〈0.05）;The complication rate of group B（0.51%） showed significant decrease compared with group A（10.6%）（P〈0.05）.Conclusion The operating time,the rate of conversion to laparotomy and complication of patients in 2009 year are decreased compared with patients in 2000 year.
