自身免疫性溶血性贫血患者外周血CD8+CXCR3+调节性T细胞数量及功能观察 被引量:6

Count and function of CD8+ CXCR3 + regulatory T cells in peripheral blood of patients with autoimmune hemolytic anemia

摘要 目的探讨CD8+CXC趋化因子受体3(CXCR3)+调节性T细胞在自身免疫性溶血性贫血(AIHA)发病中的作用。方法用流式细胞术检测2010年7—11月天津医科大学总医院血液科收治的11例初治(初治组)、11例治疗后完全缓解(完全缓解组)AIHA患者的外周血CD8+CXCR3+/CD8+T细胞比例及CD8+CXCR3+白细胞介素10(IL-10)+/CXCR3+CD8+T细胞比例,并与23名健康对照(对照组)比较。分析上述指标与CD3+CD4+细胞数量及CD5+CD19+/CD19+比值的相关性。用反转录-聚合酶链式反应(RT-PCR)技术检测外周血单个核细胞(PBMC)中CXCR3mRNA的表达水平。结果AIHA初治组外周血CD8+CXCR3+/CD8+为(39.80±19.96)%,低于完全缓解组[(58.76±14.22)%,P〈0.05]及对照组[(59.66±12.62)%,P〈0.01]。初治组外周血CD8+CXCR3+IL-10+/CXCR3+CD8+为(22.98±14.96)%,低于对照组[(38.15±17.03)%,P〈0.05]。初治组PBMCCXCR3mRNA相对表达量为(0.51-±0.19),低于对照组(1.67±1.17,P〈0.01)。外周血CD8+CXCR3+/CD8+与CD3+CD4+细胞数量及CD5+CD19+/CD19+比值均呈负相关(r=-0.571,-0.583;均P〈0.05);CD8+CXCR3+IL-10+/CD8+CXCR3+与CD3+CD4+细胞数量及CD5+CD19+/CD19+比值均呈负相关(r=-0.524,-0.523;均P〈0.05)。结论CD8+CXCR3+调节性T细胞数量减少、产生IL-10水平下降可能导致AIHA患者免疫耐受被打破,与AIHA的发病有关。 Objective "To explore the effects of CD8 + CXCR3 + T cells on autoimmune hemolytic anemia (AIHA). Methods Twenty-two AIHA patients, including 11 untreated and 11 recovered ones, and 23 normal controls were recruited from July 2010 to November 2010. The percentage of CD8 + CXCR3 + / CD8 + T cells in peripheral blood and the expression of interleukin-10 (IL-10) in CD8 + CXCR3 + T cells were detected by flow cytometry. Their correlations with the count of CD3 + CD4 + cells and the percentage of CD5+ CD19 + in CD19+ B cells were analyzed. The expression level of CXCR3 mRNA in PBMC was determined by semi-quantitative reverse transcription-polymerase chain reaction (RT-PCR). Results The percentage of CD8+ CXCR3 +/CD8 +of untreated AIHA patients was (39. 80 ±19.96)%. And it was lower than that of recovered patients [ (58.76 ± 14. 22) %, P 〈 0. 05 ] and normal controls [ (59.66 ± 12. 62) %, P 〈 0. 011. The percentage of IL-10 ± T cells in CD8 + CXCR3 +T cells of untreated patients was ( 22. 98 ±14. 96)% and it was lower than that of normal controls [ (38. 15 ±17.03)% ,P 〈0. 05]. The expression level of CXCR3 mRNA for untreated AIHA patients was (0. 51 ±0. 19) and it was lower than that of normal controls (1.67± 1.17, P 〈 0. 01 ). The percentage of CD8+ CXCR3+/CD8+ T cells had a negative correlation with the count of CD3 + CD4 + cells and the percentage of CD5 + CD19 +/CD19+ B cells (r = - 0. 571, - 0. 583,both P 〈 0. 05). So did the percentage of IL-10 + T cells in CD8 + CXCR3 + T cells (r = - 0. 524, - 0. 523, both P 〈 0. 05). Conclusion The decreased count of CD8 +CXCR3 +T cells and the lowered level of IL-10 may disturb the immune tolerance and lead to the occurrence of AIHA.
