非小细胞肺癌患者呼出气冷凝液ET-1测定的意义 被引量:8

Clinical significance of measuring endothelin-1 in exhaled breath condensate of patients with non-small cell lung cancer

摘要 目的测定非小细胞肺癌(NSCLC)患者呼出气冷凝液(EBC)中内皮素-1(ET-1)并研究其临床意义。方法按既定标准收集41例NSCLC病例(NSCLC组)及45例正常对照(正常对照组)的EBC并同时抽血分离血清,采用酶联免疫吸附法(ELISA)测定其ET-1含量。结果 NSCLC组EBC和血清的ET-1[(10.35±0.23)pg/mL、(79.62±0.25)pg/mL]显著高于正常对照组[(8.30±0.16)pg/mL、(68.21±0.35)pg/mL],差异有统计学意义(P<0.05);NSCLC腺癌组EBC的ET-1为(11.05±0.31)pg/mL,鳞癌组为(9.61±0.25)pg/mL。随TNM分期,NSCLC组Ⅱ期、Ⅲ期、Ⅳ期EBC的ET-1分别为(8.61±0.18)、(9.98±0.21)、(11.53±0.36)pg/mL,Ⅱ期与Ⅳ期有统计学差异(P<0.05)。检测EBC中ET-1作为NSCLC诊断方法的敏感性和特异性为95.12%和53.33%。结论测定EBC中ET-1有助于NSCLC的诊断和评估预后。 Objective To investigate the clinical significance of measuring endothelin-1(ET-1) in exhaled breath condensate(EBC) of patients with non-small cell lung cancer(NSCLC).Methods We recruited 41 patients with NSCLC and 45 subjects as controls in the study according to the established criteria,and collected EBC from the subjects with NSCLC and the controls.Serum samples were also collected from the two groups.ET-1 level was determined using ELISA method and statistical analysis was carried out using a software package(SPSS17.0).Results The mean level of ET-1 in EBC of subjects from NSCLC group was(10.35±0.23)pg/mL,while the level of ET-1 in serum was found to be(79.62±0.25)pg/mL.As for control group,the mean level of ET-1 in EBC and serum was(8.30±0.16)pg/mL and(68.21±0.35)pg/mL,respectively.There were significant differences between the values obtained from both groups(P〈0.05),and the value was significantly higher in NSCLC group than in control group.The level of ET-1 in EBC of subjects with adenocarcinoma and squamous cell carcinoma was(11.05±0.31)pg/mL and(9.61±0.25)pg/mL,respectively.According to the TNM stage,the value of EBC-ET-1 from subjects in stages Ⅱ,Ⅲ and Ⅳ was(8.61±0.18)pg/mL,(9.98±0.21)pg/mL and(11.53±0.36)pg/mL,respectively.As a method of diagnosing NSCLC,the sensitivity and specificity of ET-1 level in EBC were 95.12% and 53.33%,respectively.Conclusion Determination of ET-1 in EBC is particularly helpful in the diagnosis and prognosis of NSCLC.
