超声造影在前列腺癌射频消融围手术期中的初步应用 被引量:4

摘要 目的探讨超声造影在前列腺癌整体射频治疗围手术期中的应用价值。方法 8例拟行前列腺癌整体射频消融的前列腺癌患者,术前先行经直肠超声造影观察前列腺灌注情况,术中在经直肠超声造影的引导和监测下行前列腺整体射频消融,术后1~3个月内采用超声造影判断疗效和并发症。结果术前超声造影检出4例患者前列腺有高增强区域,其中2例活检高增强区域证实为前列腺癌。术中8例患者均在超声造影引导及监测下完成前列腺癌整体射频消融治疗,平均消融范围约96%。术后超声造影随访1~3个月内肿瘤无进展,并通过腔内超声造影发现1例患者发生膀胱直肠瘘并发症。结论超声造影在前列腺癌整体射频消融围手术期发挥了重要的作用。 Objective To determine the value of pert-operative contrast-enhanced uhrasound in radiofrequency ablalion of prostate cancer. Methods Eight patients with prostate cancer underwent radiofrequency ablation of the prostate glands. Contrast-enhanced transrectal ultrasound was used to assess the perfusion of the prostate glands before ablaiion, to guide and monitor the ablation, and to evaluate the curative effect as well as complications in the 1- 3 month follow-up period. Results Of four patients with hyper-perfused zones in the prostate deteeted by pre-operative eontrast-enhaneed Iransreetal uhrasound, two were confirmed to have prostate eaneer in the hyper-perfused zones. Radiofrequeney ablation of the entire prostate glands was successful in ,all 8 patients under uhrasound guidance. The average ablation volume was 96%. No tumor progression was found on follow-up nhrasound. Vesieorectal fistula was detected by intra-eavitary contrast-enhanced uhrasound in one patient. Conclusion Peri-operative contrast-enhanced uhrasound may be valuable in radiofrequency ablation of prostate eaneer.
