摘要
目的了解外科住院患者全肠外营养(TPN)使用情况,分析营养药物使用的合理性。方法对医院2010年3月至2010年12月外科住院患者的TPN处方进行统计分析。结果 112例患者接受TPN治疗共649例次,以消化道手术患者为主,占96.4%,平均用药天数为5.8d;热氮比值为100~300的处方560张,糖脂比值>3的处方418张,分别占总处方数的99.4%和64.4%,液体量基本≥1800ml,能量在1500~2038kcal之间。结论外科住院患者TPN处方设计基本合理,少数处方存在能量、氮量不足的现象。临床医师严格掌握TPN的适应证,根据患者年龄、身高体重、疾病特点、代谢情况等,制定个体化肠外营养方案,以改善患者的营养状况。
Objective To investigate the utilization of total parenteral nutrition(TPN)and analysis nutrition medicine use rationality.Methods From March 2010 to December 2010,the TPN prescription of surgery patients were analyzed.Results 112 patients received a total of 649 times treatment of TPN,gastrointestinal surgery patients accounted for 96.4%, the average number of days of drug use was 5.8 days,560 prescriptions of the ratio of calories and nitrogen between 100 to 300, 418 prescriptions of the ratio of sugar and fat greater than 3, the total number of prescriptions were 99.4% and 64.4%, the basic amount of liquid was greater than 1800 ml,the energy between 1500 to 2038 kcal.Conclusion The TPN formulation of surgical inpatients design is reasonable.There is a small number of prescription energy or nitrogen deficiency phenomenon.Clinicians should strictly control the indications of TPN,according to patient age, height and weight, disease characteristics,metabolism,etc,develop individualized parenteral nutrition programs and improve the nutritional status of patients.
