2009年某院西药门诊处方分析

Analysis of the Western Medicine in out-patient prescription in 2009

目的了解某三甲医院2009年西药门诊处方用药情况,以提高处方质量,促进合理用药。方法采用回顾性分析方法,随机抽查医院2009年1-12月西药门诊处方,将不合理用药处方进行统计分析,判断医院合理用药概况。结果共审核处方8961张,其中抗菌药物处方3151张占35.16%;不合理用药处方451张占5.03%。医院在用药与诊断不符、用法用量不合理、处方不完整、处方书写不规范、联合用药欠妥等方面存在问题。结论医院门诊用药基本合理但仍存在一定问题,应加强临床医师、药师对《处方管理办法》和药品说明书的学习,提高医师书写处方和合理用药的能力,提高药师审核处方的能力,确保患者用药安全、有效、合理、经济。

Objective To analyze the situation of irrational drug use in a third-grade hospital in 2009,and to improve the quality of prescriptions and promote the rational drug use.Methods By the way of retrospective study,randomly examined and analyzed the Western Medicine in out-patient prescription in 2009,to irrational prescriptions for statistical analysis,and determined hospital rational drug use profile.Results 8961 cases prescriptions were examined,which included 3151 cases antibacterial drug prescriptions accounting for 35.16% and 451 cases irrational drug use prescriptions accounting for 5.03%.The problems were irrational drug use existed in the mismatch of medication and diagnosis,unreasonable drug usage and dosage,incomplete and non-standard writing prescriptions,and irrational drug combination.Conclusion The drugs are rationally used in the out-patient of hospital,but there still exists some problems,which intensely asks clinicians and pharmacists to learn 《The Prescription Management Method》 and the drug instructions so as to improve clinicians′ ability in prescription writing and rational drug use and improve pharmacists′ ability in checking prescriptions,which can also ensure the patients′ safe,effective,reasonable,and economical medication.
