主动脉夹层的磁共振成像技术及临床应用

MR Imaging (MRI)Techniques and It's Clinical Application in Aortic Dissection

目的 探讨主动脉夹层磁共振成像技术及其临床意义。方法 ２５ 例不同类型的主动脉夹层患者，应用心电门控多层自旋回波序列行轴位、左前斜位磁共振成像以及左前斜位磁共振电影。结果 ２５ 例均获得满意图像，其中Ⅲ型１４ 例，Ⅱ型４ 例，Ⅰ型７ 例。在常规ＳＥ图像上可观察到破口位置、内膜片及附壁血栓，结合电影磁共振动态显示，可评价心脏及主动脉瓣功能，鉴别附壁血栓与慢血流信号，确定破口大小、数量及位置以及血流动态情况。结论 选择合适的扫描参数以及成功的应用心电门控技术是确保获得高质量ＭＲ 影像的重要因素，作为一种无创性检查方法，磁共振成像可成为诊断主动脉夹层的主要检查方法。

Objective To investigate MRI techniques and it's clinical application in aortic dissection.Methods 25 patients with different types aortic dissection were examined using ECG gated spin echo sequence and gradient echo sequence to get axial or left anterior oblique section MR image and cine MR image.Results The reliable MRI image was obtained in all the 25 cases,including type Ⅲ:14 cases,type Ⅱ:4 cases and type Ⅲ:6 cases.The entrance and exit sites,true and false channel,intimal flap and thrombus can be demonstrated on SE MR image.Cine MR image can be used to evaluate the function of heart and valve,distinguish the slow flow from thrombus in false channel,and define the size and sites of entrance and exits as well as to display dynamic blood flow in the aortic cavity.Conclusion As an invasive method,MRI have significant potential in the diagnosis and evaluation of patients with aortic dissection.The suitable scan paraments and successful ECG trigger is important factors for obtained high quality MR image.
