肝细胞生长因子和TNF-a在2型糖尿病肾病中的诊断价值

Diagnostic value of serum HGF and TNF-α in patients with nephropathy due to type 2 diabetes

目的:观察2型糖尿病肾病(DN)患者血清肝细胞生长因子(hepatocyte growth factor,HGF)和肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)的水平变化,探讨其在DN中的临床意义。方法:2006年1月至2009年12月入我院治疗的2型糖尿病患者88例(糖尿病非DN组30例、早期DN组26例、临床期DN组20例、终末期DN组12例)采用ELISA方法检测患者血清中肝细胞生长因子(HGF)和肿瘤坏死因子-α(TNF-α)含量,并与同期50例健康对照组比较。结果:(1)2型糖尿病组血清HGF及TNF-α水平显著高于健康对照组(P<0.01);(2)2型糖尿病各组血清HGF及TNF-α水平随糖尿病肾病的严重程度加重而增加,各组间差异均有统计学意义(P均<0.01);(3)2型糖尿病各组血清HGF及TNF-α水平呈显著正相关(r=0.786,0.725,0.817,0.802,P<0.05).结论:血清HGF及TNF-α水平在糖尿病肾病发病中起重要作用,并且和糖尿病肾病的分期密切相关。

Objective：To explore the changes of serum HGF and TNF-α and evaluate the clinical value of them in patients with type 2 diabetic nephropathy. Methods：From January 2006 to December 2009,88 patients with type 2 diabetes mellitus in our hospital were enrolled in the study. There were 30 cases of diabetes mellitus without DN （group A）,26 cases of early period DN （group B）,20 cases of clinical period DN （group C） and 12 cases of late period DN （group D）. We used ELISA to detect HGF and TNF-α. Fifty healthy individuals were enrolled to serve as the control group. Results：1）HGF and TNF-α levels of type 2 diabetes mellitus were higher than that of control group（P0.01）.2）The serum HGF and TNF-α levels of type 2 diabetes mellitus groups increased with severity of disease states of diabetic nephropathy. （P0.01）.3） The serum concentrations of HGF were positively correlated with that of TNF-α in every type 2 diabetes mellitus group （r=0.786,0.725,0.817,0.802,P0.05）. Conclusion：HGF and TNF-α play an important role in pathogenesis of type 2 diabetic nephropathy. Meanwhile,it is closely related to the phases of diabetic nephropathy.
