米索前列醇预防剖宫产后出血的临床应用

Clinical application of Misoprostol to prevent bleeding after cesarean section

目的：结合临床实践经验，探讨米索前列醇预防剖官产后出血的临床应用。方法：选取2008年1月-2009年12月期间，我院产科住院行选择性剖宫产的产妇120例为研究对象，采用对照研究，将其分为对照组（60例）与观察组（60例），所有孕妇均采用硬膜外麻醉，行改良式子宫下段剖宫产术。对照组应用催产素，观察组应用米索前列醇。比较两组产妇的各项指标，采用MicrosoftExcel建立数据库．运用SPSS13．0统计软件进行统计分析。所有计量资料用（均数士标准差）表示，采用t检验及Y0检验，取P〈0．05为差别有统计学意义。结果：两组产妇胎盘娩出时间、术中及术后2h的出血量比较，差异具有统计学意义（P〈0．05）；观察组有新生儿窒息2例，催产素组有新生儿窒息2例，两组新生儿窒息率比较，差别无统计学意义（P〉0．05）。观察组用药后，有4例患者出现恶0、呕吐，未经治疗数分钟后自行缓解，无发热、皮疹等不良反应。对照组未见恶口、呕吐等不良反应，差别有统计学意义（x2=4．39，P〈0．05）。结论：米索前列醇预防剖宫产后出血效果良好，且具有一定的优越性，值得临床推广应用。

Objective： To carry out Clinical application of Misoprostol to prevent bleeding after cesarean section with Clinical practice experience. Meth- ods ： Select 120 cases in our hospital In January 2008-- December 2009 as the research object. Use controlled study, divided patients into control group （60 cases） and the observation group （60 cases） , All pregnant women have epidural anesthesia, improved line of lower uterine segment cesarean section. The control group received Oxytocin, The observation group received Misoprostol. Observe and compare various indicators to evaluate the application value. Es- tabtishment of a database using Microsoft Excel, using SPSS 13. 0 statistical software for statistical analysis. UseT and X2 test, taking P ~0.05 for the difference was statistically significant. Results： The difference between two group of maternal placenta of time, intraoperative and postoperative blood loss in 2h was statistically significant （P 〈0. 05） ; observed 2 cases of neonatal asphyxia group, oxytocin group and 2 cases of neonatal asphyxia, two neonatal asphyxia group compared with no significant difference （P〉 0.05）. Observation group after treatment, 4 patients had nausea, vomiting, and relieve itself after a few minutes without treatment, no fever, rash and other adverse reactions. Control group, no nausea, vomiting and other adverse reactions, the difference was statistically significant （X2= 4.39, P 〈0.05）. Conclusion： Using Misoprostol to prevent bleeding after cesarean section can give better results, and has some advantages, is worthy of clinical application.
