摘要 目的了解机械通气患者下呼吸道细菌分布的情况。方法选取2007年至2009年入住我院呼吸重症监护室(R ICU)并行机械通气的患者,进行下呼吸道细菌培养,记录细菌培养结果。结果 1、共有212例患者入选,其中62例(占29.2%)的患者下呼吸道细菌培养为阳性。2、62例下呼吸道细菌培养为阳性的患者中,17例(占27.4%)为革兰氏阳性细菌生长,45例(占72.6%)为革兰氏阴性细菌生长。3、革兰氏阴性菌组与革兰氏阳性菌组相比,年龄,开始使用抗生素至机械通气的时间均存在差异,差异具有统计学意义(P<0.05)。结论对于机械通气的患者,应根据患者基础疾病,已治疗情况等因素判断细菌种属,合理使用抗生素。 Objective To realize the bacteria distribution of lower respiratory in the incipient stage of mechanical ventilation.Methods The patients of mechanical ventilation admitted to respiratory intensive care unit in 2007~2009 served as subjects.We did the sputum bacterial cultivation,and recorded the results of sputum bacterial cultivation.Results a.62(29.2%) patients had lower respiratory tract bacteria culture positive in the total of 212 patients.b.In those with lower respiratory tract bacterial cultures positive,17(27.4%) showed the growth of gram-positive bacteria,45(72.6%) showed the growth gram-negative bacteria.c.There were differences in patients' age and the duration between those treated with antibiotic drug and those with mechanical ventilation in the measure of gram-negative bacteria and gram-positive bacteria.The differences are statistically significant.(P0.05).Conclusion Depending on the patients' basic disease and the treatment method,rational use of antibiotics is suggested on the patients of mechanical ventilation.
