胸腔镜辅助下早期结核性包裹性胸腔积液的治疗

Study of surgical treatment assisted by VATS on early-stage tuberculous pyothorax

目的探讨胸腔镜辅助下结核性胸膜炎包裹性胸腔积液的治疗效果。方法选取2006年7月～2009年6月间34例结核性胸膜炎包裹性胸腔积液患者,行胸腔镜辅助下治疗,分析经胸腔镜治疗后的临床效果。结果全组患者胸腔镜下经术中将纤维板彻底清除,清除胸腔积液,使肺完全复张。术后给予抗结核药物治疗,随访1年无复发。结论对反复穿刺及置管引流不畅的结核性早期包裹性胸腔积液,胸腔镜下手术治疗是一种行之有效的治疗方法。

Objective To investigate the therapeutic effects of packaged tuberculous pyothorax treated with VATS.Methods 34 patients of tuberculosis packaged pyothorax diagnosed from July 2006 to June 2009 have been included in this study and the clinical effects of VATS have been analyzed.Results All patients accepted the operation.The thicker pleural,the pleural effusion was completely removed and the lung was expanded completely.All cases took the anti-TB drugs after treatment and no relapse has been found after 1 year＇s follow-up.Conclusion VATS is an effective treatment method for patients with early stage tuberculous packaged pyothorax,especially for patients after repeated puncture or drainage without any effective results.
