摘要
目的:产盐酸丁丙诺菲(沙菲)用于术后小儿PCIA镇痛效果和不良反应。方法:选择ASA,Ⅰ-Ⅱ级择期手术患儿,80例,年龄7～10岁,术后采用0.9mg盐酸丁丙诺菲+1mg氟哌利多+生理盐水43ml,配制成50ml镇痛液。PCIA模式为负荷剂量5ml,持续剂量2ml/h,单次剂量锁定时间为0.5ml/15min,镇痛时间24h,于术毕患儿清醒,将一次性镇痛泵接与静脉,并静注镇痛液5ml负荷量,随即开启PCIA开关。结果:80例患儿,采用盐酸丁丙诺菲+氟哌利多PCIA镇痛均取得满意疗效,所有患儿均未出现呼吸抑制、眩晕、嗜睡、头痛等严重不良反应,其中3例出现呕吐、2例出现恶心、1例出现皮肤瘙痒。结论:国产盐酸丁丙诺菲镇痛效果明显,不良反应轻,成瘾小,患者容易接受,可安全有效的用于术后患儿PCIA镇痛。
Objective：To evaluate the efficacy of hydrochloric acid D C Nuofei（Shafi）PCIA for postoperative analgesia in children and adverse reactions.Methods：ASA,Ⅰ ～ Ⅱelective surgery patients,80patients aged 7～10years old,after using C Nuofei Hydrochloride 0.9mg＋1 mg of droperidol＋saline 43ml,50ml analgesic preparation of liquid.PCIA mode loading dose 5ml,continuous dose of 2ml/h,a single dose of lock time 0.5ml/15min,analgesia 24h,at the completion of surgery in children awake,the one-time access and intravenous analgesia pump,and intravenous analgesia 5ml liquid load,then open the PCIA switch.Results：80patients were using hydrochloric acid D C Nuofei＋droperidol PCIA analgesia with satisfactory results,all patients had respiratory inhibition,dizziness,drowsiness,headache and other serious adverse reactions,including three cases of vomiting,2cases of nausea,1patient had pruritus.Conclusion：Domestic Hydrochloride C Nuofei significant analgesic effect,adverse reactions,addiction is small,patients with acceptable,safe and effective for postoperative analgesia in children with PCIA.
