摘要 目的探讨特殊二尖瓣反流形态与二尖瓣病变的关系。方法经彩色多普勒超声检查具有特殊二尖瓣反流形态者65例,分析其反流形态与二维图像显示二尖瓣(装置)形态、结构、启闭运动的关系。结果本文分析了三种特殊二尖瓣反流形态,均与二尖瓣(装置)病变有密切关系:Ⅰ.沿左房前内侧壁反流形态多由二尖瓣后叶脱垂及二尖瓣后叶腱索断裂引起;Ⅱ.沿左房后外侧壁反流形态的主要形态学改变是二尖瓣前叶脱垂及二尖瓣前叶腱索断裂;Ⅲ.二尖瓣膜中反流多与二尖瓣穿孔(裂)有关。结论特殊二尖瓣反流形态与二尖瓣(装置)病变关系密切;二尖瓣特殊反流形态与二尖瓣病变的二维图像特征互为指引,可提高诊断二尖瓣病变的速度和准确率。 Objective To evaluate the relations between the exceptional form of the mitral regurgitation (MR) and the the mitraI valve disease. Mettlods 65 patients with exceptional MR were Examed using CDFI to analyse and evaluate the relations between the different forms of MR and the mitral valve diseases. Results The three types of MR stream are related to the different mitral valve(MV) diseases. 1 Most of the stream of the MR towards to the anterior-medial wall of the left atrium are correlated to the MV prolapse and chordas rupture of the posterior mitral valve. 2 MV prolapse and chordas rupture of the anterior leaflet of mitral valve are responsible for the stream of the MR towards to the posterior-lateral wall of the left atrium. 3 The major cause of the stream of the MR towards to the middle of the left atrium is generally MV perforate or cleft. Conclusions The exceptional mitral regurgitation is directly correlated to the mitral valve (device) lesions. Both the CDFI of exceptional MV and the two-dimensional imaging of mitral valve (device) lesions can guide the diagmoses each other, and also can be used to increase the efficienc and to improve accuracy of the diagnoses, of the MV diseases.
