摘要 目的通过分析不同病理类型的肾上腺肿瘤的CT表现,评价CT对肾上腺肿瘤的定位和定性诊断准确性及价值。方法收集28例经手术病理证实的肾上腺肿瘤,对其CT表现进行回顾性分析。结果肾上腺腺瘤12例,表现为有功能者瘤体较小,无功能者较大,密度均匀,CT值较低,增强有不同程度强化。嗜铬细胞瘤7例,瘤体较腺瘤大,密度不均匀,以周边强化明显。髓样脂肪瘤2例,CT表现具有特征性的脂肪密度。皮质腺腺癌1例,体积较大,内见坏死区与钙化灶。节细胞神经瘤1例,密度略不均匀,内见钙化,增强仅有轻度强化。淋巴瘤1例,密度不均匀,内见坏死,不均匀强化。转移瘤4例,密度均匀,无明显坏死,轻至中度强化。结论肾上腺肿瘤CT表现中,部分具有一定特征性。通过CT平扫、增强检查,结合临床表现及相关生化检查,可以作出较准确的定位和定性诊断。 Objective To evaluate accuracy and values of CT of location and quality to adrenal tumors through analysing its CT manifestations of different pathological types. Methods Twenty-eight patients with adrenal tumors were collected and the CT manifestations were retrospectively analyzed, which were verified by operations and pathologies. Results Twelve adenomas were low and uniform density, intensify of different grade. Functional ones manifested small volumes and no functional ones had bigger volumes. Seven pheochromocytomas had bigger volumes, uneven densities and obvious intensifies in brim. Two myelolipomas had CT character containing fat density. One adrenaocortical carcinoma had big volume, necrotic area and calcify that was mildly intensified in enhancement. One lymphoma manifested uneven density, necrotic area and incoordinately intensified. Four metastases manifested even densities, no necrotic area and were mildly or middling intensified. Conclusions Some CT manifestations of adrenal tumors have characters. By CT plain and enhancing scans combining clinic and relative biochemical examinations, correct diagnosis of location and quality can be performed.
