经桡动脉行冠状动脉造影1000例并发症分析

Ananlyzing the Complications of Coronary Angiography Throngh Right Radial Approach

目的:分析经桡动脉冠状动脉造影产生并发症的原因及预防处理措施。方法:回顾2007年10月～2010年12月我院1 000例经桡动脉行冠状动脉造影的患者,记录手术并发症及预防治疗措施。结果:桡动脉痉挛94例(9.4%),男性43例(6.7%),女性51例(15%),性别比差异有统计学意义(P<0.05)。鞘管或导管拔出困难4例(0.4%);低血压15例(1.5%),严重心律失常11例(1.1%),其中室颤3例、缓慢性心律失常8例。穿刺部位血肿16例(1.6%),前臂张力性血肿5例(0.5%);桡动脉闭塞10例(0.1%)。结论:桡动脉内反复导管操作易发生痉挛。桡动脉造影极少有严重并发症,局部并发症发生率低。冠状动脉造影术前后如处理不当可能引起多种并发症,熟练的操作技术、严密的术中监测和有效的抢救措施是降低并发症发生的关键。

Objective：To Ananlyze the Complications of Coronary Angiography Throngh Radial Approach.Methods：We retrospectively reviewed data from 1000 patients who were undergone radial angiography at our institution from December 2007 to November 2010.Baseline clinical and procedural data were recorded.Results：All complications were seen in 14.6%of procedures.The incidence rate of Radial artery spasm was 9.4%.The incidence rate of Ventricular fibrillation,Low blood press,Severe forearm bleeding and hematoma,radial artery occlusion was 0.3%,1.5%,1.6%,0.1%,respectively.Conclusion：The Right radial artery is prone to spasm.Larger artery occlusion and anatomical variation are seldom.Severe complication is rare.Transradial approach for coronary intervention is a safe and elective approach so long as the patient is indicared.
