电视胸腔镜在食管良性疾病诊断和治疗中的应用

APPLICATION OF VIDEO-ASSISTED THORACIC SURGERY IN THE DIAGNOSIS AND TREATMENT OF BENIGN ESOPHAGUS DISEASES

1992 年3 月～1999 年9 月,我们在临床开展电视胸腔镜手术(VATS)300 余例次,其中食管良性疾病14例,包括食管平滑肌瘤9 例,贲门失弛缓症3 例,食管憩室1 例,食管裂孔疝1 例。手术时间90 ～210min,平均150.4min,术后平均带胸腔引流管时间1.9d,平均术后住院8.3d,中转开胸4 例,无手术死亡及严重并发症。与传统手术方法相比,该方法具有创伤小、术后进食早、术后住院时间短等优点,且暴露好,近期效果满意。

More than 300 cases of Video-Assisted Thoracic Surgery (VATS) had been performed in our department since 1992. Among these cases, there were 14 cases of benign esophagus diseases including 9 cases of esophageal leiomyoma, 3 cases of achalasia, 1 case of esophageal diverticulum and 1 case of hiatal hernia. No operative death and server complication occurred in this group. The mean operating time was 154.3 minutes (ranging from 90 to 210 minutes). The mean postoperative hospital stay was 10.9 days (ranging from 6 to 30 days), and the mean duration of chest drainage after surgery was 2.2 days. This technique has the advantage of less trauma、faster recovery、earlier feeding and shorter hospitalization, comparing with conventional thoracotomy. VATS is a safe and effective technique for benign esophagus diseases.
