电子鼻对非小细胞肺癌挥发性标志物检测的可行性 被引量:6

Feasibility of volatile markers for non-small-cell lung cancer with electronic nose

摘要 目的应用电子鼻筛选非小细胞肺癌(NSCLC)患者呼气中的挥发性标志物,并评估其用于临床诊断的可行性。方法用CAT#232-03型Tedlar袋收集20例NSCLC患者呼气样本,收集20例健康人的呼气标本作为对照。样本经预浓缩管进行浓缩,气相色谱柱进行分离,声表面波(SAW)检测标本中的挥发性有机化合物(VOCs),筛选出NSCLC患者呼气中特异性的挥发性标志物,通过C6-C22对其进行校准并定性,Mann-WhitneyU检验进行组间比较,通过受试者工作曲线(ROC)评估它们用于临床诊断NSCLC的可行性。结果癸醛、酞酸二乙酯的浓度在NSCLC者及健康志愿者呼气中的浓度存在明显差异,癸醛在NSCLC者呼气中浓度高于健康志愿者,并鳞癌中的浓度高于腺癌。酞酸二乙酯在正常人呼气中的浓度高于NSCLC患者,在鳞癌和腺癌组浓度差异无统计学意义。癸醛诊断NSCLC的灵敏性和特异性可达到95.0%与70.0%,诊断鳞癌的灵敏性和特异性达到80.0%和70.0%。酞酸二乙酯诊断NSCLC的灵敏度和特异度达80.0%和70.0%。结论癸醛、酞酸二乙酯可作为NSCLC患者呼气中特异性标志物,癸醛可能与NSCLC的病理类型有关。 Objective To screen volatile breath markers for non-small-cell lung cancer(NSCLC) and assess their feasibility for the diagnosis of NSCLC.Methods Breath samples were from 20 patient with NSCLC using Tedlar gas bags and selected 20 healthy people as control.The samples were concentrated with pre-concentration tubes and separated with gas chromatography.The volatile organic compounds(VOCs) in the Tedlar gas bags were analyzed with surface acoustic wave(SAW).Selected VOCs were distinguished with C6-C22,the levels of the selected VOCs were compared with two groups by Mann-Whitney U Test;ROC curves were used to assess the diagnostic value of these markers.Results Decanal and diethyl phthalate showed statistical significance for the median concentration differences between NSCLC and healthy volunteers.For decanal,the median concentration was higher in NSCLC than that in healthy volunteers,and the concentration of squamous cell carcinoma was higher than that of adenocarcinoma.For diethyl phthalate,the median concentration was higher in healthy volunteers than that inNSCLC.It showed no statistical significance of the concentration between squamous cell carcinoma and adenocarcinoma.Decanal had the sensitivity of 95.0% and the specificity of 70.0% between NSCLC and healthy volunteers,with the sensitivity and specificity could be achieved 80.0% and 70.0% between squamous cell carcinoma and adenocarcinoma.Diethyl phthalate had the sensitivity 80.0% and 70.0%,which showed no statistical significance of the concentration between squamous cell carcinoma and adenocarcinoma.Conclusion Decanal and diethyl phthalate could be used as volatile markers for NSCLC.Decanal might be helpful discern between squamous cell carcinoma and adenocarcinoma.
