摘要
目的 探讨精神分裂症病人各脑叶CT 值的变化及其与精神症状的相关性。方法 62 例精神分裂症病人评定阳性症状量表(SAPS) 和阴性症状量表(SANS) ,测定各脑叶CT 值,并与年龄、性别匹配的健康对照组比较。结果 病人组左额叶及两侧枕叶CT 值显著高于对照组;精神分裂症Ⅰ、Ⅱ型及混合型之间各脑叶CT 值无显著差异,不同病程病人CT 值无显著差异,但SAPS 及SANS 因子分与各脑叶CT 值的相关分析显示,妄想与右额叶及两侧枕叶、怪异行为与右额叶、阳性思维形式障碍及SAPS 总分与两侧枕叶、注意障碍与左额叶CT 值均呈显著正相关,兴趣缺乏/ 社交缺乏与右额叶显著负相关;CT 值有性别差异。结论 大脑亚微结构的异常,可能是精神分裂症某些症状的病理基础。
Objective To explore the change of CT values of lobes of cerebrum and the correlation connected with psychopathological state of schizophrenics. Methods By 62 schizophrenics to assess the Scale for the Assessment of Positive Symptoms(SAPS) and the Scale for the Assessment of Negative Symptoms(SANS); to measure the CT values of lobes of cerebrum; to use these values to compare with the healthy control group, matched with the age and sexuality.Results (1) The CT values of left frontal lobe and bilateral occipital lobes of patients' group are markedly higher than those of control group. (2) There is no apparent difference in CT values of every lobe of cerebrum among type I、II and mixed type paleophrenia and in the different disease process patients. But the pertinent analysis between the SAPS' and SANS' subscores and the CT values of every lobe of cerebrum showed, there are observable positive correlation between delusion and right frontal lobe or bilateral occipital lobes, clownism and right frontal lobe, positive thinking form or SAPS score and bilateral lobes, disturbance of attention and left frontal lobe. There is observable negative correlation between the interest or social intercourse deficiency and right frontal lobe. (3) The CT values have sexual difference. Conclusion The abnomality of submicrostructure of brain maybe the pathological basis of some schizophrenic symptoms.
