外剥内扎加Y—V皮瓣肛管成形术治疗环状混合痔

Treatment of Circumferential Mixed Hemorrhoids with the Procedure of External Dissection ＆ Internal Ligation plus Y-V Flap Anal Plasty

为探讨外剥内扎加Y—V皮瓣肛管成形术治疗环状混合痔的临床疗效，回顾性分析225例环状混合痔患者分别行外剥内扎术（对照组112例）和外剥内扎加Y—V皮瓣肛管成形术（治疗组113例）的手术治疗效果。结果显示，随访6～36个月，对照组总有效率82．0％，创口愈合时间（22．5±5．3）d，术后Ⅱ、Ⅲ度疼痛103例，术后出血15例，术后水肿50例，肛门狭窄18例，复发20例；治疗组总有效率98．5％，创口愈合时间（14．7±3．8）d，术后Ⅱ、Ⅲ度疼痛87例，术后出血5例，术后水肿12例，复发7例。结果表明，外剥内扎加Y-V皮瓣成形术治疗环状混合痔具有疗效确切、复发率低、术后并发症少、无肛门狭窄等优点。

For the aim of exploring the curative effect of the combined procedure（external dissection ＆ internal ligation plus Y-V flap anal plasty） for circumferential mixed hemorrhoids author analysed the surgical outcomes from the following two groups.controlled group in which 112 patients with this disease were subject to external dissection ＆ internal ligation,and treatment group in which 113 patients to above-mentioned procedure,i, e routine operation plus Y-V flap anal plasty,retrospectively. As results,the follow-up of 6-36 months found that in controlled group the total effective rate was 82.0% ,wound healing-time was （22.5±5.3）days,postoperative pain of Ⅱ and Ⅲ degree was in 103 cases,postoperative bleeding was in 15 cases,postoperative edema was in 50 cases,anal stenosis was in 18 cases,and recurrence was in 20 cases; while in treatment group,in turn 98.5%, （14.7±3.8）days,87 cases,5 cases, 12 cases,and 7 cases. These results show that the combined procedure has such advantages as certain curative effect, lower recurrence rate, less complication, no anal stenosis, etc.
