复方白及糊对反流性食管炎模型大鼠食管黏膜的影响

Influence of Compound Hyacinth Bletilla Paste on the Esophageal Mucosa of Acidic Reflux Esophagitis Model Rats

目的探讨复方白及糊防治反流性食管炎的作用机制。方法将Wistar大鼠随机分为假手术组、全程中药组、造模后中药组及生理盐水组,每组23只。除假手术组外,其余3组均制备反流性食管炎大鼠模型。全程中药组造模前7天起,每天灌胃复方白及糊3ml,共21天;造模后中药组造模后开始每天灌胃等量中药,共14天;生理盐水组同时灌胃等量生理盐水。分别于术后7天、14天观察各组大鼠食管下段pH、大体标本及组织学改变,同时检测食管黏膜胃泌素(GAS)、胃动素(MOT)、血管活性肠肽(VIP)、一氧化氮(NO)水平。结果 2个中药组可使模型大鼠食管下段pH升高,损伤黏膜迅速修复,使食管黏膜GAS、MOT表达增强,VIP、NO表达减少,其中全程中药组与生理盐水组、假手术组比较差异有统计学意义(P<0.05)。结论复方白及糊可能通过局部黏膜保护、调节多种胃肠道激素及神经递质,增加下食管括约肌压力,促进胃排空,对反流性食管炎起防治作用的。

Objective To explore the functional mechanism of Compound Hyacinth Bletilla Paste in the treatment and prevention of acidic reflux esophagitis.Methods The Wistar rats were randomized into 4 groups,including the sham-operation group,the whole process Chinese medicine group,post-modeling Chinese medicine group,and normal saline group,23 in each.Except the sham-operation group,the other three groups were all made acidic reflux esophagitis models.The whole process Chinese medicine group was administered Compound Hyacinth Bletilla Paste 7 days before modeling through intragastric feeding,3 ml everyday.After modeling,the whole process Chinese medicine group and the post-modeling Chinese medicine group were administered the same dose Chinese medicine;meanwhile the normal saline group was administered the same dose of normal saline through intragastric feeding.Respectively on 7d and 14d after operation,the pH of lower portion of esophagus,gross specimen,and histological changes the esophageal mucosal gastrin（GAS）,motilin（MOT）,vasoactive intestinal peptides（VIP）,and nitric oxide（NO）levels in all groups were detected.Results The two Chinese medicine groups could make the pH of lower portion of esophagus increased,make the injured esophageal mucosa quickly repaired,make the expression of GAS and MOT of esophageal mucosa enhanced,and make the expression of VIP and NO reduced,of which,the effect of whole process Chinese medicine group was more significant,and in comparing with normal saline grown and sham-operation group there was statistical difference（P0.05）.Conclusion Compound Hyacinth Bletilla Paste functions to treat and prevent the acidic reflux esophagitis through protecting the local mucosa,regulating various gastrointestinal hormones and neurotransmitters,and increasing the pressure of lower esophageal sphincter,and promoting the gastric emptying.
