摘要
患者女,47岁。头面部及左肩部反复出现红色斑片、溃疡十余年伴左颈部及腋下结节2年。结核菌纯蛋白衍生物(PPD)试验阳性。颈部淋巴结穿刺示:上皮样细胞、多核巨细胞、淋巴细胞组成的结核结节,中央可见干酪样坏死。皮损组织病理示:真皮层可见不规则上皮样细胞聚集,大量淋巴细胞浸润,少量多核巨细胞。诊断:溃疡性狼疮合并淋巴结结核。
A 47-year-old female presented with recurrent erythema and ulcers on the head, face, right neck and left shoulder for more than ten years. Purified proteinderivative （PPD） was positive. The paracentesis of lymph nodes showed the tuberculoid nodules was composed of epithelioid cells and multinucleated giant cells and lymphocytes, there was caseating necrosis in the center. The histopathology revealed intense mixed infiltrate of lymphocytes and epithelioid ceils and a few multinucleated giant cells in the derm. The diagnosis of ulcerativelupus vulgaris complicated by lymph nodes tuberculosis was made.
