慢性充血性心力衰竭与血清CA125水平的相关性研究

Research of the relativity between chronic congestive heart failure and the level of serum-CA125

目的:探讨慢性充血性心力衰竭(CCHF)患者血清CA125与心功能之间的关系。方法:将63例CCHF患者按NYHA分级标准分为心功能Ⅱ、Ⅲ、Ⅳ级组,另外随机选择27例正常人作为对照组。采取微粒子酶免疫化学发光法(META)测定血清CA125水平,通过超声心动图测定左心室内径及射血分数(EF)。结果:CCHF患者各组的血清CA125水平均比对照组的升高(P<0.05、P<0.001)。CA125水平升高与心功能的恶化程度成正比,两组比较,差异有统计学意义(P<0.05)。CCHF组LVMI、LAVI高于对照组,Ⅳ级组高于Ⅱ级组,Ⅲ级组高于Ⅱ级组;Ⅲ、Ⅳ级心功能组LVEF低于Ⅱ级组和对照组;心脏异常患者的血清CA125水平明显比心功能正常患者的增加(P<0.001)。心脏扩大组与心功能正常组比较,血清CA125水平显著增加(P<0.001)。结论:CCHF患者血清CA125浓度增高,升高水平与CCHF患者心脏扩大呈正相关,血清CA125水平可作为判断CCHF患者心功能严重程度和影响预后的一项指标。

Objective： To discuss the relation ship of the serum-CA125 in the patients with chronic congestive heart failure patient and the heart function.Methods： According to the criterion of NYHA,63 cases of patients with CCHF were divided into heart function grade Ⅱ,heart function grade Ⅲ,heart function grade IV group,and randomly chosen 27 cases health people for the control group.Used the META to check the level of serum-CA125,through the ultrasound cardiogram to check the bore size of the left ventricle and the ejection fraction（EF）.Results： The level of CCHF patients′serum-CA125 in each heart function group was higher than that of the control group（P0.05,P0.001）.A rise in the level of serum-CA125 and the dangerous level of the heart function had the positive correlation,there was a significant difference in the two groups（P0.05）.The LVMI and LAVI in CCHF group were higher than those of the control group,the grade Ⅳ heart function group was higher than the grade Ⅱ heart function group,the grade Ⅲ heart function group was higher than the grade Ⅱ heart function group,the LVEF in the grade Ⅲ,Ⅳ heart function group were lower than the grade Ⅱ heart function group and the control group.The level of CCHF patients′ serum-CA125 in each group was higher than that of the control group（P0.001）.To compare the cardiac dilatation group and the health heart function group,the augment of the level of serum-CA125 had highly significant（P0.001）.Conclusion： The consistency of serum-CA125 in patient with CCHF is high,the level of the heightening and the CCHF patient′s cardiac dilatation have the positive correlation,so the level of serum-CA125 is a guide line to judge the order of the heart function severity and affecting the prognosis.
