记忆合金聚髌器结合Herbert螺钉治疗髌骨纵形骨折临床研究

Clinical study of treatment of longitudinal patella fracture with shape memory alloy patellar concentrator and Herbert screws

目的探讨形状记忆合金聚髌器结合Herbert螺钉内固定治疗髌骨纵形粉碎性骨折的临床疗效。方法本组髌骨纵形粉碎性骨折19例,男12例,女7例,年龄18～56岁,平均32.4岁。其中车祸伤11例,摔伤8例。均采用记忆合金聚髌器结合Herbert螺钉内固定治疗。结果 19例患者均获得随访6～38个月,平均19个月。全部病例均获得骨性愈合,术后平均7.2周时患侧膝关节屈伸功能达到健侧水平。根据Bostman临床评分系统评估,优15例,良4例,优良率为100%。结论记忆合金聚髌器结合Herbert螺钉内固定治疗髌骨纵形粉碎性骨折,复位满意,固定牢靠,是治疗伴有纵裂的髌骨粉碎性骨折的有效方法。

Objective To investigate the clinical effect of comminuted fracture of patella which longitudinal fracture lines including treated with shape memory alloy patellar concentrator and Herbert screws.Methods Nineteen patients of comminuted fracture of patella which longitudinal fracture lines including were retrospectively analyzed.There were 12 males and 7 females with an average age of 32.4 years（range 18 to 56 years）.Eleven cases were caused by road traffic accidents,8 cases by a fall onto the knee. All the cases were accepted the surgical treatment with shape memory alloy patellar concentrator and Herbert screws fixatioa Results The mean period of follow up was 19 months（range 6 to 38 months）.All the cases obtained bone union in average 7.2 weeks（from 4 to 10 weeks）.There were no complications such as infection,skin necrosis and bone nonunion.According to Bostman system,the results of clinical evaluation showed excellent in 15 cases,and good in 4 cases.Conclusions Application of shape memory alloy patellar concentrator and Herbert screws as an effective option can get satisfactory reduction with reliable fixation and will be one of a better choice for fixation of comminuted patella fracture which longitudinal fracture lines including.
