自体血液回输技术在外伤性脾破裂手术中的临床应用

Clinical application of tautologies blood transfusion for the traumatic splenic rupture

目的:探讨自体血液回输技术在外伤性脾破裂手术中的临床意义,评估其临床应用的安全性和有效性。方法:选取2006年1月～2010年1月我院外科68例外伤性脾破裂致腹腔内出血量超过600ml的患者,所有回收血液经过抗凝、滤过、清洗、浓缩后回输,并记录回输的血量,观测患者术前、术中、术后第2d的血常规、凝血功能变化。结果:68例外伤性脾破裂患者手术均顺利完成。术中回输自体血70520ml,平均1763ml。68例患者在术中、术后不再输库存血;两组术后第2d的血常规均无显著性差异。术后血液检测凝血功能等均接近正常;无一例发生输血不良反应。结论:自体血液回收技术是一种简便、实用、安全、有效、经济的方法,能有效减少术中血液丢失,血液回输后效果良好,无输血反应,术后并发症少。它能缓解血源不足,减少异体输血及并发症的发生,特别是在暂时无库存血的情况下,更是一种抢救伤病员的有效措施,值得临床推广和应用。

Objective：To explore the clinical significance of tautologies blood transfusion for the traumatic splenic rupture, and estimate the safety and effectiveness of clinical application. Methods：68 cases with traumatic splenic rupture were chosen from January 2006 to January 2010, and all leaded to amount of bleeding in the abdominal cavity more than 600 ml, all the blood strained through anticoagulation, clean, concentrated back, and record the loss of blood to lose, the patient before surgery, and observation in 2d and on the blood of the general function and blood. Results：68 cases with traumatic splenic rupture were success in the surgery. Get back the lost blood from oneself was 70520ml ml, the average was1763 ml. 68 cases don’t use the blood from hospital before of after no longer;After two days, all have no significant gender difference. After the blood test blood functions are close to normal ;No case in the blood transfusion had reaction. Conclusion：The tautologies blood transfusion skill is a simple, functional, safe, effective and economical way, can be effectively reduce the loss of blood and the return blood is good, without a blood transfusion. The complication is less after the surgical operation. It can alleviate the source of blood transfusion and complications and pockets, especially for the blood stock in no circumstances, is a best way to save the heart people, it is worth being popularized and application in clinical otolaryngology.
