87例老年腰椎管狭窄症的围手术期护理

PERI-OPERATIVE NURSING FOR 87 ELDERLY PATIENTS WITH LUMBAR SPINAL STENOSIS

目的探讨老年腰椎管狭窄症患者全麻下行腰椎管减压融合内固定手术围手术期的护理体会。方法回顾性分析总结2008年1月～2010年5月我科收治的87例于全麻下行腰椎管减压融合内固定手术治疗的老年患者的护理资料。结果 87例均顺利完成手术,手术时间为110～230 min,平均时间为(175±24)min。术中出血200～1 400 ml,平均450.0 ml。全部病例患者均获得随访,时间为8个月～1.5年,平均14.3个月。术中硬膜囊撕裂3例,术后出现脑脊液漏。术中无神经损伤,术后无神经症状加重。术后伤口均Ⅰ期愈合;无死亡病例;无严重并发症发生。术后随访症状体征改善,疗效满意。结论加强老年腰椎管狭窄症患者全麻下行腰椎管减压融合内固定手术围手术期的护理是使患者安全、顺利完成手术的保证,且可以减少相关并发症,同时正确的护理指导可加快患者康复。

Objective To investigate the peri -operative nursing for elderly patients with lumbar spinal ste- nosis undergoing lumbar spinal anesthesia, surgical decompression and internal fixation. Methods A retrospective analysis of nursing data of 87 cases from January 2008 to May 2010 in our department who were treated under general anesthesia in lumbar spinal canal decompression and internal fixation. Results The patients were successfully opera- ted. 87 cases completed an average 14.3 months follow - up from 8 months to 1.5 years. The wounds are healing I , no deaths, no serious perioperative complications. Conclusion It is insurance for patients＇ safety and success of op- eration to enhance peri - operative nursing for elderly patients with lumbar spinal stenosis undergoing lumbar spinal anesthesia, surgical decompression and internal fixation. Otherwise, it can reduce correlated complications and cor- rect nursing guiding will accelerate patients＇ rehabilitation.
